DETERMINATION OF NONSIGNIFICANCE

review of a completed environmental checklist and other information on file
with the lead agency. This information is available to the public on request.

boundaries of the site(s). Provide a legal description, site plan, vicinity

related to this checklist.
The project is iocated on the west side of Killion Road, north of the
intersection with Yelm Avenue, in Section 24, Township 17, Range
IE, Parcel No. 21724120404.

B. ENVIRONMENTAL ELEMENTS
1. Earth

The steepest slope on site is 3%.

SCS lists soils on site as Spanaway gravelly sandy loam.
d. Are there surface indications or history of unstable soils in the
immediate vicinity? If so, describe.
There are no surface indications of unstable soils in the
immediate vicinity.
e. Describe the purpose, type and approximate quantities of any filling
or grading proposed. Indicate source of fill.
There will be approximately 1,500 cubic yards of imported fill
and approximately 3,000 cubic yards of cut.
f. Could erosion occur as a result of clearing, construction, or use? If
so, generally describe.
Erosion is always a possibility on construction sites. The
highest potential for erosion will occur during the grading
process for the project. See Item h below for proposed control
methods.
g. About what percent of the site will be covered with impervious
surfaces after project construction such as asphalt or buildings?
Approximately 50% of the site will be covered with impervious
surfaces, including stormwater ponds.
h. Proposed measures to reduce or control erosion, or other impacts
to the earth, if any:
An erosion control plan will be developed in accordance with
the City of Ye1m standards. This plan will be reviewed and
approved by the City. Erosion control fencing and other
devices will be used to ensure no materials leave the site.

landscaped and revegetated.
2. Air
a. What types of emissions to the air would result from the proposal
(i.e., dust, automobile exhaust, odors, industrial wood smoke)

During construction emissions to the air urill be released from
heavy equipment and construction staff vehicles. Follouring
construction, emissions from staff and visitors will be
released on site. All emission quantities are anticipated to be
urithin normal ranges for projects of this type.

your proposal? If so, generally describe.
There are no off-site sources of emissions or odor that u~ili
affect this proposal
c. Proposed measures to reduce or control emissions or other impacts
to air, if any:
All heavy equipment urili be fitted with required emission
control devices.
3. Water
a. Surface Water
1) Is there any surface waterbody or wetland on or in the immediate
vicinity of the site (including yeaz-round and seasonal streams,
saltwater, lakes, ponds)? If yes, describe type and provide names.
State what stream or river it flows into.
Yelm Creek is approximately 2200 feet northeast of the
project boundary. Thompson Creek is located approximately
3000 feet west of the project boundary.
2) Will the project require any work over, in, or adjacent to (within 300 I
feet) the described water? If yes, please describe and attach
available plans.
No work will be done within 300 feet of Yeim Creek or
Thompson Creek.
3) Estimate the amount of fill and dredge material that would be
placed in or removed from surface water or wetlands and indicate

the area of the site that would be affected. Indicate the source of fill
material.
No fail wilt be placed in or removed from surface water or
wetlands.
4) Will the proposal require surface water withdrawals or diversions?

diversions.

b. Groundwater:
1) Will groundwater be withdrawn, or will water be discharged to

No groundwater will be withdrawn as a result of this project.
This site will be served by City of Yelm water and S.T.E.P.
sewer. Stormwater will be treated and infiltrated back into
the ground.
2) Describe the underlying aquifer with regard to quality and quantity,
sensitivity, protection, recharge areas, etc.
The entire City of Yelm is located in an area of extreme
aquifer sensitivity.
3) Describe waste material that will be discharged into or onto the
ground from septic tanks or other sources, if any (such as:
domestic sewage; industrial byproducts; agricultural chemicals).
Treated stormwater will be infiltrated into the ground. All
stormwater treatment and infiltration facilities will be
designed in accordance with City of Yelm standards.

c. Water Runoff (including storm water):
1) Describe the source of runoff (including storm water) and method of
collection and disposal, if any (include quantities, if known). Where
will this water flow? Will this water flow into other water? If so,

Runoff will be generated from the rooftop and parking area.
The stormwater system on site will be designed in accordance
with City of Yelm standards.
2) Could waste materials enter ground or surface waters? If so,
generally describe.
It is unlikely that waste materials could enter ground or
surface waters as a result of this project. Treatment facilities
have been designed to reduce this risk as much as possible.
d. Proposed measures to reduce or control surface, ground and runoff
water impacts, if any:
All stormwater systems will be designed in accordance with
the City of Yelm's standards. Treatment that meets or
exceeds these standards will be provided prior to infiltration
to the ground. All plans will be reviewed and approved by the
City of Yelm prior to construction.
4. Plants

manufacturing, transportation, etc.
The assisted living center will use electricity and natural gas
for lighting, heating and cooling.

adjacent properties? If so, generally describe.
This project will not affect the potential use of solar energy by
adjacent properties.

The building will be constructed in accordance with Uniform
Building Code and Washington Energy Code standards.

No threats to environmental health will occur as a result of
this project.

The project is located near Ye1m Avenue, the main arterial
through Yelm. Standard traffic noises at levels typical of an
urban environment exist in the project area.

with the project on a short-term or a long-term basis (for example:
traffic, construction, operation, other)? Indicate what hours noise
would come from the site.
Short term noise will be generated during construction
activity. This will primarily be composed of equipment noise
during clearing and grading, and construction noise while the
building is being built.
3) Proposed measures to reduce or control noise impacts, if any:
Construction will take place during daylight hours only.

87 assisted living and congregate care units will be provided.
23% of the facility will be available to Medicaid subsidized
patients.

10. Aesthetics
a. What is the tallest height of any proposed structure(s), not
including antennas; what is the principal exterior building
material(s) proposed.
The tallest height of any proposed structure is 39' 9" feet,
measured to the top of the cupola. The principal exterior
building material proposed is Hardiplank.
b. What views in the immediate vicinity would be altered or
obstructed?
There is a view of Mt. Rainier to the east from the site. Views
from the north, west and south of the project will have their

view altered by converting the project property to a developed
site.
c. Proposed measures to reduce or control aesthetic impacts, if any:
The site will be landscaped in accordance with the standards
of the City of Yelm. A 15 foot Type 1 Zandscaping buffer will
be provided between the assisted living facility and the
residential zoning to the north. Trees, shrubs and ground
cover will be planted to create an aesthetically pleasing
appearance. The project will be designed to a residential
scale, using residential-type materials in order to blend with
the adjacent residential uses.
11. Light and Glare
a. What type of light or glare will the proposal produce? What time of
day would it mainly occur?
Lighting urill be designed so that it is directed on site and to
minimize the amount of light spilling onto adjacent
properties.
b. Could light or glare from the finished project be a safety hazard or
interfere with views?
Lighting will be designed to comply with ail safety standards
and to minimize glare. No safety hazards will be created by
the project.
c. What existing off-site sources of light or glare may affect your
proposal?
There are no off-site sources of light or glare that would
affect this proposal.
d. Proposed measures to reduce or control light and glare impacts, if
any:
AIi fixtures on site will be designed to minimize or eliminate
Zght spillage onto other properties and the city right-of-way.
12. Recreation
a. What designated and informal recreational opportunities are uz the
immediate vicinity?
Nisqually Valley Golf Course, Yelm Park and Cochran Park are
located within one mite of the project site.

recreation and a Looped concrete sidewalk for walking.

next to the site.
No evidence of historic, archaeological, scientifc, or cultural
importance has been observed on site.

None are proposed.
14. Transportation
a. Identify sidewalks, trails, public streets and highways serving the
site, and describe proposed access to the existing street system.
Show on site plans, if any.
The project will access Killion Road by two private driveways.
Frontage improvements on Killion Road will be constructed by
the project proponent, to include a 12-foot sidewalk with tree
grates.
b. Is site currently served by public transit? By what means? If not,
what plans exist for transit service?
Intercity Transit serves Yelm via Ye1m Avenue. Buses are not
expected to travel into the site at this time. A van will be
provided by the assisted Living facility as transportation for
the residents.

many would the project eliminate.
No parking spaces would be eliminated. 49 parking spaces
are being proposed for the completed project.
d. Will the proposal require any new sidewalks, trails, roads or streets,
or improvements to existing sidewalks, trails, roads or streets, not
including driveways? If so, generally describe (indicate whether
public or private).
As part of the project, frontage improvements will be required
to be constructed on Killion Road. These improvements will
be consistent with the "typical pedestrian oriented street
section" and "commercial collector" standards of the City of
Yelm's Development Guidelines.
e. Will the project use (or occur in the immediate vicinity ofl water,
rail, or air transportation? If so, generally describe.
The project will not use or occur in the immediate vicinity of
water, or air transportation. The Burlington Northern
Railroad Line is located approximately 3500 feet west of the
site.
f. How many vehicular trips per day would be generated by the
completed project? If known, indicate when peak volumes would
occur.
According to the Ye1m Transportation Facility Charge
Ordinance, the project uri11 generate .11 new pm peak hour
trips per assisted Living dwelling unit and .21 new pm peak
hour trips per congregate care unit, or a total of 12 p. m. peak
hour trips.
g. Proposed measures to reduce or control transportation impacts, if
any.
In accordance urith the Ye1m TFC ordinance, the project will
pay a transportation facility charge of $750 per pm peak
trip.

The project is not anticipated to result in a measurat
increase in demand for public services. Occasional police

appearance. The project will be designed to a residential
scale, using residential-type materials in order to blend with
the adjacent residential uses.
