040200 Construction Comment Letter 3 11-28-06 City of Yelm Community Development Department 105 Yelm Avenue West P.O. Box 479 Yelm, WA 98597 November 28, 2006 Bob Bromberg 13224 83rd Ave E Puyallup, WA 98373 RE: Discovery Counseling – SPR-04-0200-YL Construction Inspection Letter Mr. Bromberg: On Monday November 27, 2006, the City of Yelm had received a call and an office visit by you, for a final acceptance of your site. I spoke with you in the office and informed you that I had not yet been contacted by you for a final inspection of the completed work on your site on. I also assured you that I would get out Monday afternoon and complete the inspection. Jim Gibson and I visited your site that Monday afternoon. I made my recommendations to Jim on what I found to be incomplete or unacceptable work. Tuesday November 28, 2006. You came back into the office to discuss the final acceptance of your site. I informed you that there were concerns with the bio-swale and other drainage issues that were not completed or done per plan. Furthermore I explained to you that you would need to take care of the above mentioned per plan. We had also recommended you speak to your Design Engineer and seek his advice for completing the work per plans and to coordinate a meeting between all of us on site, if necessary to resolve any and all issues that you do not understand. You had called me sometime later that morning, to let me know that you had spoken to your Design Engineer Bill Turner and he had explained to you and made recommendations on what you have to do to complete your site per plans. When we visited the site on Monday November 27, 2006, we found the following discrepancies: The Bioswale was not installed per plan. The soil and hydroseed were burmed up behind the curb cuts and would not allow the storm water to drain properly. This also created an erosion problem on 2nd Ave. The Landscape areas in the parking lot were not completed per plans either, thus creating insufficient drainage. After reviewing the discrepancies and discussing them internally in the office, it is decided that these issues will have to be completed per plans before we will accept this project as being complete. Please review your plans and complete the work to the plans. You may want to call me for an inspection of your work before you hydroseed or Saud the bio-swale area. After this has been inspected and accepted, we can then proceed with the finalizing of your project site. If you have any questions or concerns, please do not hesitate to contact me at steveh@ci.yelm.wa.us or 360-458-8423.
