Box 479 Yelm, WA 98597   August 10, 2006 Jeri Lynn Brock 14538 McIntosh Lane SE Tenino, WA 98589 Re: Brock Offices, 211 Yelm Avenue West The Community Development Department inspected your site listed above and find the following items to be completed prior to issuing a certificate of occupancy. Based on the Conditions of Approval dated May 2006, the following items are incomplete: Condition # 1 required deferral of frontage improvements and dedication of right of way. Attached please find the required forms. You will need to have a surveyor provide the legal description for the right-of-way dedication. Condition #5: To be inspected by Water Department, you will be notified if they have any concerns. Condition # 7: Your parking area needs to be painted with stripes marking the parking stalls, and the proper ADA painting and signage. Condition #10 Refuse: At this time, you do not have an area for a commercial dumpster. We are assuming that you are planning to use residential garbage cans. They must be kept out of site from the right-of-way during non pick-up days. When the items listed above are complete, please contact Gary Carlson, Building Official at (360) 458-8407 to request the certificate of occupancy for your business.
