City of Yelm Daily Inspection Form 2 Yelm Creek Top Rock & Paving City of Yelm CONSTRUCTION PROGRESS AND INSPECTION REPORT Date: 9/19/06 Day: Tuesday Time: 9:30 a.m.  Project Name: Yelm Creek Project Number SUB-05-0239-YL  Project Description: Subdivision Contractor's Name: RGD Rainier General Development  1. Brief Weather Summary: (On earthwork jobs include soil conditions) Cool, cloudy, Soil conditions are Compacted crushed surfacing top rock.  2. Contract Time: No. Days Charged To Date Last Working Day Charged (Date)  3. Description of Construction Phases (Include items such as clearing, grading, drainage, base, paving, surface, utilities, curb and gutter, sidewalk, etc.) Contratractor placed crushed surface top rock but did not call for an inspection for top course. I noticed Lakeside loading up equipment and asked if they were going to Yelm Creek. Lakeside rakeman told me he didn’t know, only they were going somewhere else in town. I hurried over to Yelm Creek at around 9:30 a.m. I spoke to Roger to confirm Lakeside Paving at Yelm Creek. I informed him to that they needed to call me and schedule a proof roll on the crushed surface rock. He had a 12 yard dump truck he was loading with conrete and asphalt pieces, I told him when that was full we could use it for a proof roll, he agreed. When proof rolling there were 3 areas that I had some concern with and would need to be compacted with the roller again. It appeared to me that there was sepapration in the rock and fines on the crushed. I asked Roger if the rock was stockpiled or if they brought it out on the day it was spread. He said it was from the approved pit stock pile but it was the last of it. Proof roll complete, Lakeside on site mobbing equipment at 9:40 a.m. Lakeside began paving on re-rolled area at culdesac. Lakeside Density Tester had also noted this condition. I asked for test results. Roger had a question on the end of sidewalk temp asphalt ramp. He would like to concrete it. I informed him it must be 5’ and put in per plans. He also talked to me about the solid pipe on the storm vault being 10’ shorter than plan calls for, as approved by Jim Gibson ? I will ask jim about this. I called Tony Bloom to let him know that I was not called for a top rock inspection and I just happened to show up on site before Lakeside and had a proof roll done and made comments as to the results of proof roll. Top rock was not completely ready and needed to be rolled and compacted again. I would recommend we get an extended warranty on this road. Yelm Creek getting 2” of asphalt. Crystal Springs half street improvements are getting 3” to 4” of Asphalt. Lakeside completed paving at Yelm Creek and Crystal Springs Rd. crossing and de-mobed around 2:50 p.m. Tony Bloom on site, we talked briefly on how the project was going. I told him I do have some concern on the roadway loose rock, he said yea, I hope it don’t fail. I said I hope not. I will monitor the roadway time to time and document any changes if it happens.  4. Summary of Field Testing (Note failing tests and any retests. Condition of material) Contractor re-rolled and compacted top course on Yelm Creek, due to loose rock.  5.
