cc-061301 Vet Clinic Pre-Application Meeting Civil Comments Date June 13, 2001 ***These comments are preliminary in nature and are not intended to represent final comments and or requirements for the City of Yelm. Until a complete application is made, the planning and/or public works department can only attempt to inform the applicant of general requirements as they appear in the form presented by the applicant at the time of pre-submission. Proponent: Dr. Jenifer Preston Project: Convert existing home into veterinary clinic Project Loc: 1314 Yelm Ave. Water:  TC \l1 " The proposed site is not currently connected to the City of Yelm’s water system. An existing 10” diameter water main is located in Yelm Ave. You will be required to connect to the water main and install a water meter. The water meter will need to be sized by your civil engineer. The water meter fee is $300.00 after your water tap and meter setter are in place. Water hook-up fees are based on a consumption rate of 240 gallons per day and are charged at a rate of $1,000/240gpd (fee subject to change) inside city limits. At the time of application submittal you will be required to submit water usage calculations provided by a licensed civil engineer. The water fees will be calculated using the calculations provided. TC \l1 " If there is a well on-site, you will be required to decommission the well in accordance with Department of Ecology and relinquish any water rights to the city. Sewer:  TC \l1 " The proposed site is not currently connected to the City of Yelm’s STEP sewer system. An existing 4” STEP sewer main is located in Yelm Ave. You will be required to connect to the City’s STEP system. This will require a new step sewer tank and pumping system. I have attached a detailed sketch showing what may be required. Sewer hook-up fees are based on a consumption rate of 240 gallons per day and are charged at a rate of $4,850/240gpd (fee subject to change) inside city limits. At the time of application submittal you will be required to submit water usage calculations provided by a licensed civil engineer. The sewer fees will be calculated using the calculations provided. TC \l1 " If there is a on-site sewer septic system you will be required to remove the system per the Department of Health guidelines. Fire:  TC \l1 " Fire protection to the buildings must be provided per the Uniform Building Code. There may be existing fire hydrants on this site, however, additional hydrants will be required to meet current standards. This will be determined upon further review of the civil plans.  TC \l1 " Stormwater: TC \l1 " Developments with over 5,000 sq ft of impervious surface are required to provide stormwater water quality facilities per the DOE Stormwater Manual. If your impervious surface area is less than 5,000 square feet you will only be required to provide stream bank erosion control. Preliminary Stormwater Report and conceptual plan must be submitted with site plan review application. TC \l1 " Street Improvements: TC \l1 " Frontage improvements are not required for this project at this time. However these improvements may be required if your building improvements are greater then 60% of the assessed building value. Street Lighting: TC \l1 " Nothing to report. ADA Requirements: The American Disabilities Act required that facilities are ADA accessible. Facility improvements will have to meet current code.
