Box 479 Yelm, WA 98597   November 3, 2004 Design Systems 717 22nd St SW Puyallup, Washington 98371 RE: 512 Yelm Ave W – Parking Lot Resurfacing - Project No. CIV-04-0023-YL Dear Larry: This letter serves as your approval for the above referenced project. Please review the following conditions and inspection requirements for this project. Conditions of Approval: The applicant will be responsible for maintaining erosion control measures and not impacting adjacent properties in any way. The approved plans show the limits of the approval. Any change or deviation from these plans without written approval from the inspector will not be allowed. A preconstruction meeting will be required before construction can begin. 4 sets of approved plans need to be submitted to the Community Development Department when scheduling the preconstruction meeting. Please allow 1 week for scheduling the precon meeting. The general contractor and its subcontractors must have a City of Yelm business license. The may be purchased at Yelm City Hall. Inspection Requirements: The following inspections will be required. Subgrade inspection before paving, curbing and striping inspection, stormwater trench before backfilling and before covering. Water and sewer departments will be inspecting the site to maintain their facilities and insure there protection. Please notify the inspector 2 days before paving is to begin.
