Box 953 GCL# EDWAREL848D3 360-701-3562 TENINO, WA 98589 Specializing in Asbestos, Lead, Mold Abatement and Interior Demolition EDENVIROLLC@GMAlL.COM Letter Report Limited Scope AHERA Survey 505 E Yelm Ave Yelm, WA 98597 1.0 INTRODUCTION This letter report presents the results of the asbestos survey performed by Edwards Environmental Services, LLC, at 505 E Yelm Ave Yelm, WA 98597 on January 22nd, 2019, and the laboratory results of bulk samples of suspect building material collected during the site visit. 2.0 SCOPE OF WORK The scope of the services proposed and performed included the following: ® Perform a field survey to evaluate suspect building material and document whether or not suspect building materials contain the presence of asbestos. Collect bulk samples of friable and non -friable suspect asbestos -containing material for laboratory testing. 6 Prepare this survey report including recommendations for asbestos abatement action. 3.0 PROCEDURES All persons participating in asbestos inspections have received training as detailed in the Asbestos Hazard Emergency Response Act (AHERA), 29 CFR Part 763 and are accredited asbestos building inspectors. Sample locations for this survey were chosen in a random fashion, with emphasis being placed on obtaining samples of each type of accessible suspect material, while minimizing damage to the material being sampled. Samples were collected by carefully removing small portions of the suspect material in a non-abrasive manner. If possible, samples were collected from existing damaged areas. Immediately after collection, samples were placed in plastic containers. Sample containers were then placed in a large reseal able plastic bag for transportation to the laboratory. Data pertinent to each sample such as date, sample number, material description, and material condition was recorded on a field data sheet. Asbestos bulk samples were sent under appropriate chain -of -custody procedures to Asbestos Northwest, LLC in Federal Way, Washington for analysis. The samples were analyzed using polarized light microscopy/dispersion staining techniques in accordance with U.S. EPA Method 600/M4-82-020. Detection limits for this type of analysis are approximately one percent by volume. Materials containing one percent asbestos or more are considered to be asbestos - containing materials. Asbestos Northwest participated in the National Voluntary Laboratory Accreditation Program (NVLAP) for quality control procedures. Laboratory analysis reports are attached. 4.0 MELD OBSERVATIONS Building being surveyed is residential home. Construction is stick built, wood siding, trussed roof system with 3 tab roofing, and a full basement. Building was constructed in 1946 and has approximately 1000 square feet living space. Interior finishes were textured gypsum wallboard on walls, popcorn ceiling texture, ceilings tiles, carpeting throughout with exception to the bathroom, kitchen, and basement area. These areas had rolled vinyl sheet goods, 12 x 12 and 9 x 9 floor tiles. Suspect building materials included in scope were sheet rock with joint compound and texture, popcorn ceiling texture, ceiling tiles, fiber board, sheet vinyl flooring, floor tiles, cove base glue, window glazing, roofing and duct tape associated with the heating system. All three out buildings had no suspect building materials with the exception of window glazing. All suspect building materials were identified and inspected. These materials were then paced into one of two categories, either suspect for presence of asbestos or non -suspect for the presents of asbestos. Roofing was asphalt 3 -tab shingles, and building was insulated with fiberglass batt, and other cellulose blown in insulation. No pipe insulation or other TSI's were found. Suspect building materials were identified as suspect for presence of asbestos and bulk samples were collected. Each material is listed with field identification number, material description, samples(s) number and laboratory result. 5.0 FIELD OBSERVATIONS Edwards Environmental Services collected seventeen samples of materials considered suspect for the presence of asbestos. All materials were sampled following AHERA protocol. Of the seventeen samples taken ten tested positive for asbestos that were sample # BE 01 sheet vinyl in the bath room, sample # BE 02 9 x 9 floor tiles and mastic, samples # BE 04, 05, 06, & 07 all popcorn ceiling texture, samples # BE 09, 010, & 011 dry wall with joint compound 2 % chrysotile( recommend to have these samples point counted) and sample # BE 012 duct tape associated with the heating system. All out buildings had no suspect building materials with the exception of window glazing and that has tested negative for asbestos. 600 RECOMMENDATIONS A copy of this report must be provided to any employee or contractor conducting renovation or demolition activities at the subject property. Regulated ACM are required to be handled in accordance with Washington State Regulations prior to any demolition, renovation, or remodeling that would disturb these materials. Washington State Department of Labor and Industries require that the abatement be performed using a certified asbestos abatement contractor, workers, and the supervision of an AHERA supervisor. The only exemption of this is for the residential owner occupiers to perform removal of asbestos materials them self only. The requirements for handling, packaging, and disposing of asbestos containing materials can be found in WAC Chapter 296-62, Volume I, Part 1-1 ACM with less than 1% asbestos is not required to be abated by certified contractors or workers. However, ACM with less than 1 % does require all workers to have asbestos awareness training prior to handling or abatement work. Exception to this is the full structure demolition with machinery. This Asbestos Survey Report has been prepared for the exclusive use of Prairie Park Holdings, and those hired to assist in structure renovation. A reasonable effort was made to identify all suspect containing building materials (ACBNI) within the interior of the unit only; however, this does not imply a guarantee that all possible asbestos - containing materials were identified because certain building materials may have been inaccessible at the time of the inspections. Newly identified materials should be sampled and confirmed as non -asbestos containing before activities resume. We trust this report meets your current requirements. The six types of asbestos fibers are chrysotile, amosite, crocidolite, tremolite, anthophyllite and actinolite. A sample which contains more than 1% asbestos is considered positive and is defined by the EPA as an Asbestos Containing Material (ACM). The samples were analyzed in accordance with EPA method 600/R-93/116 and 600/M4-82-020. The analyst used a stereomicroscope to visually inspect the sample to determine homogeneity and material descriptions. The sample was then viewed under a polarized light microscope to determine the presence and percentage of asbestos and non -asbestos fibers. After analysis is complete, all paperwork will be filed together, and kept in a secure locked filing cabinet away from other clients and laboratory staff.
