DNS SEPA NO: 8322 DETERMINATION OF NONSIGNIFICANCE Proponent: Easthaven Villa II Description of Proposal: Request for a Conditional Use Permit, to allow the construction of an additional 20 assisted living units, and associated facilities addition to the existing permitted assisted living facility. Location of the Proposal: 311 Cullens Road, Yelm, WA 98597 Section/Township/Range: NE ¼, Section 24, Township 17N, Range 1E, W.M., Lot 1 of SS-0427 Threshold Determination: The City of Yelm as lead agency for this action has determined that this proposal does not have a probable significant adverse impact on the environment. Therefore, an environmental impact statement (EIS) will not be required under RCW 43.21C.030(2)(c). This decision was made after review of a completed environmental checklist and other information on file with the lead agency. This information is available to the public on request. You may appeal this determination to the Yelm City Council, at above address, by submitting a written appeal no later than 5:00 p.m., October 4, 2002. This DNS is not a permit and does not by itself constitute project approval.
