8336 Punchlist for final 102105 City of Yelm Community Development Department 105 Yelm Avenue West P.O. Box 479 Yelm, WA 98597   October 21, 2005 Fran Sherrill Yelm Senior Center PO Box 474 Yelm WA. 98597 Dear Fran: This letter is a follow-up to our site visit today regarding the status of the Yelm Senior Center (CUP-03-8336-YL). The Community Development Department has conducted a site visit and the following list of comments will need to be addressed. Building Final This is the certification from the Building Official that the building (everything within 5 feet of the foundation) meets the requirements of the Uniform Codes. The building punch list has been prepared by Gary Carlson, the City’s Building Official. The items on this punch list are: The back door landing must be at least 4’ in width. The crawl space access must have covers installed on them. The address must be posted on the building. Temporary Certificate of Occupancy This is the permission issued by the City to use and occupy the building, even though some of the requirements of the building code or land use approval may not have been completed. In the case of Yelm Senior Center, the City will issue a temporary certificate of occupancy when the building has been given final approval and all the items on the punch list have been complete. The handicapped signs must be installed on the ADA stalls. A sidewalk must be installed between the ADA stalls and existing sidewalk. An accessible path must be provided for the right side of each van accessible stall in to the building. The old guy wire on the pole must be removed. Finish driveway approach Back fill behind all sidewalks. A rock berm needs to be created on the S. side of Yelm Cr. to prevent the HDPE pipe bedding/backfill from eroding. Replace incorrect wire (too small) from the Pump Control Panel to the tank "J" box. Schedule 80 PVC is required for use in the discharge line as it goes through the grommet in the tank riser. The End of Line clean out and all gate valves must have a concrete pads to meet the Yelm Spec. The 1" S.S. ARV pipe must be insulated. Rubber protectors need to be installed between the HDPE pipe and the steel supports on the bridge. The riser for the ARV must be set and backfilled. All HDPE, and SS piping must be bedded, and backfilled. The remote disconnect switch located adjacent to the Pump Control Panel must be fitted with the correct lockable cover. The Pump Control panel must have the appropriate "Yelm" sticker on the door. The Pump Control Panel, and all wiring must be approved by L&I Per the City of Yelm Tech. Spec. the wires used both from the building panel to the Control Panel and from the Control Panel to the STEP tank must have different colors to enable trouble shooting future problems. During walk through I was unable to access the main breaker panel to verify breaker sizes Pump vault riser has been installed upside down. Several options exist for correcting this. Wires shall be "made up" in vault "J" box with heat shrink butt connectors. The City will consider issuing an ‘event’ certificate of occupancy for short term (one-day) events in the buildings if the building has been given final approval and progress on the remaining items is continuing. Certificate of Occupancy This certificate is issued when all building and land use issues are complete. Prior to issuance of the certificate of occupancy, the City requires all punch list items to be completed and approved. The City will hold the amount from the CBDG grant payout until this requirement is met and the certificate of occupancy issued. The follow item must be completed before the Certificate of Occupancy will be issued. Finish side walk along 103rd St. Complete the frontage improvements. A refuse container will required if a dumpster is going to be used. The water and sewer easements must be submitted, reviewed and recorded. The stormwater maintenance manual should be submitted by the engineer. Landscape must be installed and approved.
