Box 73156 Puyallup, Washington 98373 Johns Meadow Subdivision City of Yelm Project No. SUB-04-0175-YL Mr. Laudermilk: Our department has completed a final walk thru on the above mentioned project. The following are a few minor items that were discovered during this walk thru, these items must be completed to assure in finalizing the project. These items are listed as follows: Community Development Department: Bring ground level up to sidewalk grade both front and back of sidewalk on all locations of Plat. Remove all Garbage and Debris through out Plat. Clean up rock piles on various locations of Plat. Grade all lots and place top soil behind sidewalks. Finish Fencing. Landscaping: 1. Hydroseeding required within plat and in planter strip along Middle Road. 2. Complete perimeter fencing along Middle Road. 3. Complete perimeter fencing on west property line. 4. All planter strips must contain topsoil. 5. Complete fencing around Catherine Dotson’s property. 6. Tree on lot 37 needs to be at least 5 feet away from any utility boxes if possible. Public Works Department Comments: Sewer Department - Sewer system: Lots 1 & 2 need toning wire into the Carson box from the main in the street. Lots 15 & 16 need toning wire into the carson box from the main in the street Lots 17 & 18 need toning wire into the carson box from the main in the street Broken Carson Box lid must be replaced for lot 31 Broken Carson box lid must be replaced for lot 36 Air Release Assembly vault lid must have S.S. screws as supplied by Orenco installed to secure the lid. Broken Carson box lid must be replaced for lot 50 Water Department - Water system, cross control: 1.  Lot 1. – Raise meter box to final grade – Ensure all meter boxes are at final grade when landscape are finished. 2.  The irrigation meter at the storm pond needs the Double Check Valve Assembly installed and test before the water is left on to the meter and the meter box needs to be backed filled around. 3.  The air release at the end of Greenbrier Court is in the wrong location, needs to be moved to other side of the meter setters as per print. 4.  Lots 10 and 11 – Move the meter box back and straighten back of meter setters. 5.  The meter setter at lot 31 needs to be raised. 6.  The meter setter at lot 32 is installed backwards.           7.  The air release at the end of Greenbrier Street is leaking. 8.  Lots 12 and 13 – Move the meter box back and straighten back of meter setters 9.  The fire hydrant at the end of Greenbrier Street is too high. 10.The fire hydrant at Cochrane   and Middle Road is too high. 11. The irrigation meter Middle Road and Cochrane needs the Double Check Valve Assembly installed and test before the water is left on to the meter and the meter box needs to be backed filled around. 12. Make sure all the Stroz’s fitting screws are tight. 13. Backflow assembly must be tested by a certified backflow tester and a copy of the report sent to the water department for the CCS files. 14. The Bacterial test results have been received and are good. 15. Is the well on the existing house going to be pulled and if so the water department needs a copy of the well being pulled report? Street Department – Sidewalk, Curbing and Gutter: 1.  Clean all sidewalks 2.  Remove all plastic from extruded domes 3.  One crack on Lot 11, marked in Red 4.  Work on all grades front and back of walk 5.  Dirt on walk at Lot 5, 1 foot deep plus 6.  Trim tree branches on front side of walk Please address each one of these punch list items. Do not call to schedule a re-inspection of these items until all punch list items are 100% complete.
