installation of these poles.

Mr. Robin / Mr. Harrington
I have attached a copy of the standard plan for slip bases. The max
height for the anchor bolt is 4 inches and a 1/8 inch clearance the next
plate (see note 13 on attached standard plan), and slip washers need to
be installed in the proper location. If you have any questions about
the installation of the slip bases please contact me. Thank you for
your quick response to this issue.

I will have a crew out this Thursday the 12th to make the Anchor Bolt
Projection 4" above grade. I will have a street light crew out either
tomorrow afternoon or Wednesday to fix the slip washer problem and
complete total installation.

Please see the email below. These luminaries need to be corrected
immediately to minimize any State and City exposure to accident
liability. It would be helpful if a schedule could be provided
indicating when this work will start. Also some of the luminaries on SR
510 installed by Intelite are incorrect as well and need to be
reinstalled. Steve if you have any questions or concerns please contact
me.

Hi Jay :
Cliff and I went out to SR 507 and looked at the luminaires that
Intellite installed . We found the slip washers were located on the
wrong side and the base bolts are too high ,bolts above slip plane. All
slip plains need to be a maximum of 4 inches above grade.
