GENERAL NOTES (STREET CONSTRUCTION)
1. ALL WORKMANSHIP AND MATERIALS SHALL BE IN ACCORDANCE WITH CITY
DF YELM STANDARDS.
2. THE CONTRACTOR SHALL BE RESPONSIBLE FOR ALL TRAFFC COMROL IN
ACCORDANCE WITH MUTCD. PRIOR TO DISRUPTION OF ANY TRAFFIC,
TRAFFC CONTROL PWJS SHALL BE PREPARED AND SUBMITTED TO THE
CITY FOR APPROVAL NO WORK SHALL COMMENCE UN11L ALL APPROVED
TRAFFIC CONTROL IS IN PUCE
3. ALL CURB AND GUTTER, STREET GRMES, SIDEWALK GRADES, AND ANY
OTHER VERTICAL AND/OR HORIZONTAL ALIGNMENT SHALL BE STARED BY
AN ENGINEERING OR SURVEYING FIRM CAPABLE OF PERFORMING SUCH
woRK.
4. WHERE NEW ASPHALT JOINS EXISTING, THE EXISRNG ASPHALT SHALL BE
CUF TO A NEAT VERTICAL EDGE AND TACKED WITH ASPHALT EMULSION
TYPE CSS-1 IN ACCORDANCE WITH THE STANDARD SPECIFlCARONS. THE
NEW ASPHALT SHALL BE FEATHERED BACK OVER IXISTING TO PROVIDE
FOR A SEAL AT THE SAW CUT LOCATION AND THE JOIN SEALED WITH
GRADE AR-4000W PAVNG ASPHALT.
5. COMPACTION OF SUBGRADE, ROCK, ANO ASPHALT SHALL BE IN
ACCORDANCE WITH THE STANDARD SPECIFICATIONS.
6. SUBGRADE INSPECTION SHALL BE PERFORMED BY AN APPROVED TESTING
FlRM AND FORMS SHALL BE INSPECTED FDR LINE AND GRADE BY AN
ENGINEER BEFORE POURING CONCRETE.
7. CERTIFICATION AS TO CONFORMANCE SHALL BE PROVIDED TO THE CITY
PRIOR TO ACCEPTANCE OF WORK.
B. THE COMRACTDR/DEVELOPER SHALL FURNISH AND INSTALL ALL
PERMANENT SIGNING.

3. A PRECONSTRUCTIDN MEETING WILL BE HELD WRH THE Cltt OF YELM,
DEPARIMEM OF COMMUNITY DEVELOPMEM, PRIOR TO THE START OF
coNSTRUCnoN.
4. ALL STORM CONVEYANCES AND RETENTION/DETENTION AREAS WILL BE
STAKED FOR GRADE AND ALIGNMENT BY AN ENGINEERING OR SURVEYING
FlRM CAPABLE DF PERFORMING SUCH WORK.
5. STORM DRNN PIPE WILL MEET THE FOLLOWING REQUIREMENTS. SUBMITTAL
WILL INCLUDE THE PIPE TYPE AS ALLOWED PER CffY DF YELM
DEVELOPMENIT GUIDE7JNES.
6. SPECIAL STRUCTURES, OIL/WATER SEPARATORS, AND OUTLET CONTROLS
WILL BE INSTALLED PURSUANT TO PLANS AND MANUFACTURER'S
RECOMMENDATIONS.
7. PROVIDE TRAFFC CONTROL PWJ(S) AS REQUIRED IN ACCORDANCE WITH
MUTCD.
8. CALL UNDERGROUND LOCATE AT 1-800-424-5555 A MINIMUM OF 48
HDQRS PRIOR TO ANY EXCAVATIONS.
9. WHERE CDNNECTIDNS REDUIRE 'FIELD VERIFlCARONS,' CONNECTION
POINTS WILL BE EXPOSED BY CONTRACTOR AND FffiINGS VERIFIED 48
HOURS PRIOR TO DISTRIBUTING SHUTDOWN NOTICES.
10. All STORM LINES AND CATCH BASINS WILL BE HIGH-VELOCITY CLEANED
AND PRESSURE TESTED IN ACCORDANCE WITH DMSIDN 7 DF THE
STANDARD SPECIFICATIONS PRIOR TO PAVNG IN CONFORMANCE WRH THE
ABOVE-REFERENCED SPECIFlCAlIONS. (SEE NOTE 1.) HYDRANT FLUSHING
OF LINES IS NOT AN ACCEPTABLE CLEANING METHOD.
11. TESTING OF THE STORM MNN WILL INCLUDE TELEVISION INSPEC110N OF
THE MAIN BY THE Cltt AT THE COMRACTOR'S IXPENSE IMMEDIATELY
PRIOR TD TELEVISION INSPECTING, ENOUGH WATER WILL BE RUN DOWN
THE LINE SO R COMES OIfT THE LOWER MANHOLE AND THE LINE IS
FLUSHED CLEAN. ACCEPTANCE OF THE LINE WILL BE MADE AFTER THE
TELEVSION INSPECTION TAPE HAS BEEN REVIEWED AND APPROVED BY
THE INSPECTOR. A WATER TEST DF ALL MANHOLES IN ACCORDANCE WfTH
YELM STANDARDS IS ALSO REQUIRED. TESTING WILL TAKE PLACE AFTER
ALL UNDERGROUND U1ILBIES ARE INSTALLED AND COMPACTION OF THE
ROADWAY SUBGRADE IS COMPLETED.,
