I arrived at Al’s Welding to perform special inspection and testing services scheduled for visual
welding inspection. Testing performed as follows:
I inspected the fillet welds for the following structural members:
BEAMS MARKED:
2 each – 3A; 2 each – 4A.
COLUMNS MARKED:
2 each -2B columns; 2 each -2A columns; 4 each -1A columns.
The welds are visually acceptable as per AWSD1.1-06. The welds are placed in accordance with
the approved shop drawings. Welding performed by a WABO certified welder using Triple 7
E7018 electrode, FCAW process.

Inspector Mark Robinson arrives on site, 8:20 am, for inspection of epoxy dowel placement, Overcast,
57 degrees
Contractor installs #5 reinforcing dowels into the existing floor slab to connect to the addition floor
slab. Bars are placed to comply with detail 7/S4.1 and to match slab reinforcing notes. Also placed are
three dowel bars per thickened footings (six total bars).
Materials:
#5 x 14” dowel bars; Hilti-150 epoxy, expiration date 12/2008.
Procedure:
The holes for the dowels have been drilled with a minimum embed depth of 8”. All holes
are then prepared with each drilled location blown out, brushed and re-blown with 100-psi pressurized
air through a tube extending to the back of the drilled hole until no further dust or debris leaves hole.
Dowels are placed with a bleed-out of epoxy upon completion to ensure full encasement of dowel.
Inspector also notes that the slab preparation complies with slab notes, with #4 bar placed at 16” on
center each way midway into slab, vapor barrier and perimerter insulation in place. Slab placement
tentatively scheduled for Thursday.

Inspector Mark Robinson arrives on site, 7:50 am, for inspection of forms, reinforcing, and concrete
placement. Sunny, 67 degrees
Contractor places concrete for the addition exterior footings. Inspector reviews plan notes on sheet
S2.1 and typical wall and column footing details on S4.1. All areas are approved for placement.
Concrete is placed by pump at 4.5” maximum slump at discharge and is vibrated for
consolidation. One set of compressive strength test specimens is made on the placement of 16
yards and stored in the field away from the work area for later pickup.
Field Note:
As noted in field report #8, July 31, the contractor has drilled and epoxied three #5 dowel
bars (alongside the #4 dowel bars) into the existing footing (each side) to attach to reinforcing in the
new addition footings.

concrete mix on sheet S1.0, footing plan notes on sheet S2.1, and canopy footing detail 9/S4.1. All
areas approved for concrete.
Concrete is placed by truck chute at 4.0” maximum slump at discharge and is vibrated for
consolidation. One set of compressive strength test specimens is made on the placement of 6.5
yards and stored in the field cure box away from the work area for later pickup.
