Youngs Teriyaki 12-16-16 TC EH/ December 19, 2016 Young Park, Owner 10027 Dotson Street SE Yelm, WA 98597 RE: OWNERSHIP CHANGE APPROVAL; YOUNG’S TERIYAKI; (folder# 16-111698) Dear Mr. Park: As a result of an ownership change request, our department conducted an initial assessment on October 25, 2016 for the above-mentioned food establishment located at 1010 E Yelm Ave #B, Yelm, WA. Based on the results of the initial assessment, it required several follow-up assessments due to the number of compliance issues we had observed during the initial assessment. Fortunately, the follow-up assessment conducted on November 9, 2016 and December 13, 2016 show tremendous progress and improvements with the food establishment. As a result of these improvements, we approve this ownership change with a few conditions. Please see the following conditional items are understood and followed as you continue with your food establishment operation: All sushi making shall be done inside the kitchen/food preparation area and NOT behind the front counter until the correct type of refrigeration is provided (i.e. pull out drawers or a food preparation top case refrigerator) to store all the ingredients for the sushi. The owner is expected to always use the test strips or pH meter to verify and monitor the correct pH level (maintain 4.6 or less if not hot holding or using time as control). A HACCP plan (Hazardous Analysis Critical Control Point) will likely be required in the future since you are using acidification process to prepare the sushi rice and it requires a variance per the retail food code. The owner agrees to purchase a large enough tub to use as the third compartment with the two compartment sink in case the automatic dishwasher no longer operates. The owner agrees to maintain a service contract to confirm the dishwasher is in good repair and working properly. The owner agrees that the finish wall throughout the kitchen is in need of a fresh coat of paint that is washable. Enamel gloss paint is the better paint inside kitchens or install fiberglass reinforced panels (FRP) directly over the wall for a cleaner and durable finish. Provide our department with an updated and correct food establishment plan that reflects the final look and equipment layout now that the changes and improvements have been made. Young’s Teriyaki December 19, 2016 Page Two In addition to the above items, please acknowledge and understand the following items as you continue with your business: Any expansion of the menu, addition or change of equipment, or remodel of the establishment requires review by this Department. The food flow has been reviewed and approved. If there are any significant changes to the food flow, please notify our Department to discuss and provided revised food flows. We observed during the last follow-up assessment on December 13, 2016 that the farthest restroom had already been converted into a dry storage room after an in-person discussion you had with Ron Kemp, Building Officer with the City of Yelm Building Department on November 9, 2016 at the restaurant. Unfortunately, this change of use with the restroom was completed prior to receiving a letter from Mr. Kemp dated November 14, 2016 indicating a different occupancy load for one restroom than what was originally discussed during his visit. To clarify the maximum occupant load for a restaurant with one restroom, you will need to eventually discuss this building issue further with the City of Yelm Building Department. This letter and the establishment file reflect items discussed during plan review. You will be responsible for meeting all requirements of Article II, Rules and Regulations of the Thurston County Board of Health Governing Food Service and the Washington State Retail Food Code Chapter 246-215 Washington Administrative Code. Our records indicate that your establishment is in the following billing category with the corresponding square footage. If our records are incorrect, please let us know. Category Square footage High 1,200 To complete the approval process for the ownership change, please pay your pro-rated 2016 annual food establishment permit fee and all plan review fees associated with this ownership change. The payment can be made in person, check through the mail, or over the phone with a credit/debit card (with a nominal processing fee). Please pay all fees within 5 days after receipt of this letter. We appreciate your cooperation, effort and time you have invested to get your food establishment in compliance and making necessary behavior changes to improve food safety practices and sanitation. Although a re-inspection was conducted separately due to a substandard inspection on October 25, 2016 with the same day as the final assessment, we may return in approximately 30-45 days to conduct another routine inspection.
