Box 479 Yelm, WA 98597 July 9, 2007 Allen Seay Wal-Mart Stores 2001 Southeast 10th Street Bentonville, AR 72716-0550 Dear Allen: The Community Development Department has been coordinating civil punch list items prior to your planned opening on July 18th. While I believe many of these items have been noted by either your contractor or engineer of record, I wanted to make sure everything is in one place for your ease of reference as you prepare for your grand opening. The following issues need to be addressed prior to issuance of a certificate of occupancy for the Yelm Wal-Mart. SEWER ?? At station 117+00 a concrete pad is needed on the valve box. This pad should be 6” thick by 24” square. ?? The Air Release Assemblies at station 11+00 needs an insulated lid, screws in the lid and toning wire. ?? The Air Release Assembly at station +70 needs an insulated lid, screws in the lid and a traffic bearing ring and cover. ?? The End of Line Clean-Out at station 130 +10 needs to have the blow-off and toning wire brought up as high as possible and the lid needs to be labeled “Sewer” not “Water” ?? At station 167+25 (Yelm Avenue East) the End of Line Clean Out (including the toning wire) needs to be extended up as high as possible. Rocks need to be removed from this stand pipe and a 3’-4’ valve extension added to the valve. ?? At station 53+00 the gate valve stand-pipe seems to be separated and the valve nut cannot be accessed. This needs to be corrected. ?? The power disconnect must be drilled to allow the use of a City of Yelm pad lock when the disconnect is in the “ON” position. ?? The Pump Control Panel must be equipped with a surge arrester pursuant to City of Yelm specifications. Page 2 of 3 ?? The Carson (valve) box at SR 507 (Yelm Avenue East) must be raised to finish grade. ?? The Public Works Department – Sewer Division must be supplied with test results from a certified Man-Hole Vacuum tester for the testing done to the manholes on site. ?? All traffic bearing lids 30” or larger must be aluminum per City of Yelm Specifications. ?? Due to the installation and lid size, the green lid from the ‘solids’ end of the STEP tank could not be removed. ?? The “J” Box in the STEP Tank must have the lid secured with stainless steel Phillips head screws, rather than “square” drive screws. ?? A written tank warranty will be required. ?? The pump system installed is a “duplex” system (two pumps) but only one “onoff” float was installed. A float for each pump is required. WATER ?? All on-site and off-site issues with the potable water infrastructure have been addressed. CIVIL ?? Improvements to SR 507 and the SR 510 Yelm Loop must be inspected and approved by the Washington State Department of Transportation, including all paving, striping, and the traffic signal, which must be operational before opening. ?? Improvements to 103rd Street must be inspected and approved by Thurston County. ?? The corner located at SR 507 and SR 510 Loop needs to be completed. ?? All plastic packaging needs to be removed from extruded domes. ?? The sidewalk on SR 507 from the guard rail to SR 510 Loop needs to be brought up to the concrete surface area. ?? The broken sidewalk panel adjacent to the tire and lube exit, marked with red spray paint, must be replaced. ?? The broken curb panel adjacent to the tire and lube exit, marked in red spray paint, must be replaced. ?? All sidewalk areas need to be cleaned. ?? All street signage must be installed. ?? The remaining sections of the perimeter wall must be installed. ?? After the parking lot is striped, confirmation is required that the required number of parking stalls, including the ratio of compact stalls to regular stalls, have been Page 3 of 3 installed and that the required emergency vehicle parking stall within 75 feet of door has been striped and signed. ?? All required landscaping must be installed. ?? The bus shelter near the intersection of SR 507 and SR 510 must be installed and confirmation provided that the bus stop is ADA compliant. ?? The corner enhancement must be complete. ?? A financial guarantee must be in place for the maintenance of the stormwater system, perimeter and parking lot landscaping and the building must be in place. ?? The chain link fence on SR 507 must be painted greet to match the street lights. DOCUMENTATION The following items are not required for the opening of the store, but within 45 days of the issuance of a certificate of occupancy. ?? Water line and sewer line and STEP tank easements must be recorded, including a general access easement across the property to access the City owned improvements covered by the easements. ?? Bill of Sale for all water and sewer system infrastructure to be dedicated to the City, including water mains, sewer trunk lines, and STEP tanks. ?? Dedication of all water and sewer system infrastructure to be maintained by the City, including water mains, sewer trunk lines, and STEP tanks. ?? The stormwater maintenance and operations agreement must be recorded. ?? As-built plans must be submitted to the Community Development Department. While City staff will continue to monitor your progress, it would be appropriate for you to schedule a final inspection when you believe that all punch list items have been addressed, several days prior to your scheduled opening, if possible.
