Goroch 2106 Pacific Ave. Suite 300 Tacoma, WA 98402 Re: Today’s Dental - City of Yelm Project Number - SPR-07-0135-YL Review No.3 Site Construction Documents Mr. Goroch: The City of Yelm has completed the third review of Today’s Dental construction documents. Your application included: Construction Plans Landscape Plans Step Tank Calculations We have reviewed the plans for compliance with the City of Yelm Development Guidelines, the DOE Stormwater Manual and general standards of civil engineering practice. The following comments need to be addressed before the plans may be approved for construction. Our comments are summarized below: Storm: I did not see any indication of what storm water management manual was used for this project.  I assume that the 2005 D.O.E. Storm Water Management Manual was used for this project, since the storm water treatment devices used are considered UIC’s in which would trigger the 2005 D.O.E. manual to be used for storm water design.  Please clarify. Step Sewer: The step tank will have to be traffic rated, with no parking over tank per sewer dept. previous request. Submittals are usually submitted with step tank calcs at time of civil plan submittal for review and approval. Note 6 on Sheet Notes for sheet C 14.01 sheet 7 of 22 calls out the wrong detail callout (C 04.12 Detail “A”). Sheet # 11.01 “Phase 1& 2 demolition plan” Note #17 denotes existing STEP to be decommissioned. Sheet # 31.01 Phase 3 Demolition Plan Note #7 denotes Protecting existing Septic Tank and Sanitary Pipe. What actually is going to happen and when? It appears that the new tank is going to be installed, the neighboring business hooked up to the new tank and the old tank decommissioned and abandoned. Is this assumption correct? Existing STEP tank must remain fully functional until such time as the new STEP tank is fully functional and ready to receive wastewater. The existing STEP Tank at Today’s Dental is NOT H-20 load bearing rated and must be protected from being driven over or parked on, prior to being decommissioned. Sheet c14.01 Utility Note #6 calls out for sewer lines to be SDR35 unless otherwise specified Side Sewer labels call out schedule. 40 pvc. Side Sewers need to be SDR35 and Pump service lines need to be Schedule 40 PVC. Submittals must be received and approved by the City of Yelm Sewer Dept. for the new tank. Depth of bury for the new tank must be 5 feet or less from the invert elevation of the inlet to the rim elevation of the highest rim. Sheet C14.01 Sheet note 11 discusses connecting proposed sewer line to existing Grease interceptor outlet line…The neighboring business also has a side sewer not served by the grease trap. All lines feeding to the existing STEP tank must be served by the new step tank. Existing 2” sewer pressure line is a pressure main. It is to be left intact along with the end of line clean-out in the proximity of the existing STEP tank. Sheet C14.01 sheet note 12 calls for providing a 2” check valve assembly on the existing sewer pressure line. This is not needed nor desired please leave existing sewer pressure main alone with the exception of tapping for the service line for the new tank. Sheet C14.01 sheet note #6 The Sewer Department does not understand the requirement that the service line run uphill to the main. We have not seen that requirement before. Please move pump control panel to closer proximity to the tank, near the Hose Bib would be ideal. Landscape and Irrigation: Civil plans show 21 parking stalls proposed. A minimum of 22 are required per the land use approval. Please provide a detailed corner enhancement plan as part of the landscape plans. Please provide a detail of the proposed refuse enclosure. Refuse enclosures located between or adjacent to buildings and a public way shall include an 8 foot masonry wall consistent with the building. Materials and a 5 foot landscape buffer to screen the wall. The located and access shall be approved in writing by Lemay prior to civil plan approval. Type I landscaping along the northwest property line shall include a 6 foot wood fence in addition to the plantings. A right of way dedication and waiver of protest are required for the deferral of frontage improvements for Yelm Avenue, Solberg and Jefferson. They are required to be submitted prior to civil plan approval. Please review and revise the plans to address the above referenced comments. When the project is resubmitted you will need to submit 4 sets of plans, 2 stormwater reports. The project engineer should include a written response with the resubmittal, indicating how all the review comments above have been addressed or responded to.
