11. Give brief, complete description of your proposal, including the proposed uses and the-size of the
.project and site. There are several questions later in this checklist that ask you to describe
certain aspects of your proposal. You do not need to repeat those answers on this page.
The project will be located upon two City parcels. The proposed project includes the
construction of a new one story dental clinic (size approximately 4530 SF), with associated
site development and improvements to accommodate required parking, landscaping,
utilities and potential off-site and street frontage improvements.
12. Location of the proposal. Give sufficient information for a person to understand the precise
location of your proposed project, including.a street,address, if any, and section, township, and
range, if known. If a proposal would occur over a range of area, provide the range or boundaries
of the site(s). Provide a legal description, site plan, vicinity map, and topographic map, if
reasonably available. You need not duplicate maps or detailed plans submitted with any permit
applications related to this checklist.
The Project Site consists of two individual parcels: 21724142300 & 21724142200. The two
parcels are bordered to the southwest by Yelm Ave, to the southeast by Solberg St. and to
the northeast by Jefferson Ave.
Main site address is 502 Yelm Ave W and 107 Solberg St, Yelm, WA. Three adjacent
parcels to the northwest are also owned by the applicant and all or part of these are being
considered for future redevelopment.
B. ENVIRONMENTAL ELEMENTS
Earth
a. General description of the site (circle one):

b. What is the steepest slope on the site (approximate percent slope)?
The site is basically flat with slopes ranging from 0 to 3%.
What general types of soils are found on the site (for example, clay, sand, gravel, peat,
muck)? If you know the classification of agricultural soils, specify them and note any
prime farmland.
The site is listed as Spanaway gravelly sandy loam on Thurston County's GeoData
Center. Within the limits of the soils testing that was done, the Geotech Report
states that there is an upper sod and topsoil layer up to a maximum of 1 foot thick
overlaying a black ash layer with gravel to a depth of 2 '/2 - 3 feet. Under the ash
layer was gravel glacial outwasli to a depth of about 10 feet.
Are there surface indications or history of unstable soils in the immediate vicinity? If so,
describe.
There appears to be no evidence of unstable soils in or near the site. Refer to
Geotechnical Engineering Report, dated January 4, 2007.

The site will be graded to channel stormwater runoff into catch basins, prepare for
building pads, strip organic material to make a suitable base forthe parking lot and
drive aisles. .
~f. Could erosion occur as a result of clearing, construction, or use? If so, generally
describe.
Erosion is not anticipated as a direct result of clearing or construction activities on
the subject site. However, approved industry standard methods for controlling
erosion will be in place and every effort will be taken to assure that there are no
off-site impacts if erosion does occur.
g: About what percent of the site will be covered with impervious surfaces after project
construction such as asphalt or buildings?
It is estimated that 75% of the site will be covered with impervious surfaces.
h. Proposed measures to reduce or control erosion, or other impacts to the earth, if any: nl~

City of Yelm. These include a construction entrance, silt fences around the.
perimeter of the site and catch basin protection.
2. Air
a. What types of emissions to the air would result from the proposal (i.e., dust, automobile
exhaust, odors, industrial wood smoke) during construction and when the project is
completed? If any, generally describe and give approximate quantities if known.
Construction vehicles and equipment will be a potential source of exhaust
emissions and dust during construction. Once the building is occupied employees
and customer automobile exhaust will be the main source of emissions. There are
no proposed uses in the building that emit significant smoke or odors that would
need fo be mitigated. ~
b. .Are there any off-site sources of emissions or odor that may affect your proposal? If so,
generally describe.
Off-site sources of emissions will be the similar uses on the adjacent commercial
properties, along with trafficon Yelm Avenue.
c. Proposed measures to reduce or control emissions or other impacts to air, if any:
Dust will be controlled through watering during the drier months of construction
as needed. No other measures are planned.
3. Water
a. Surface Water
1) Is there any surface water body or wetland on or in the immediate vicinity of the site
(including year-round and seasonal streams, saltwater, lakes, ponds)? If.yes, describe
type and provide names. State what stream or river it flows into?
None known.
2). Will the project require any work over, in, or adjacent to (within 300 feet) the described
waters? If yes, please describe and attach available plans.

No surface water will be withdrawn or diverted as part of this project.

b. Groundwater:
1) .Will groundwater be withdrawn, or will water be discharged to groundwater? Give
general description, purpose, and approximate quantities if known.
Treated stormwater runoff will be discharged into the ground via underground
infiltration facilites. All stormwater runoff will be infiltrated. Runoff will pass
through a treatment vault with media cartridges that has been approved by the
Washington State Department of Ecology as a basic water quality best
management practice (BMP). Please refer to the attached Preliminary stormwater
Report prepared by BCRA.
2) Describe the underlying aquifer with regard to quality and quantity, sensitivity, protection,
recharge areas, etc.
stormwater runoff from the developed project will be infiltrated to recharge the
aquifer beneath the project. Prior to entering the infiltration system the water will
be treated Runoff will pass through a treatment vault with media cartridges that
has been approved by the Washington State Department of Ecology as a basic
water quality best management practice _(BMP). Please refer to the attached
Preliminary stormwater Report prepared by BCRA.
3) Describe waste material that will be discharged into or onto the ground from septic tanks
or other sources, if any (such as domestic sewage; industrial byproducts; agricultural
chemicals).
No waste material will be discharged. into the ground.
c. ,Water Runoff (including storm water):
1) Describe the source of runoff (including storm water) and method of collection and
disposal, if any (include quantities, if known). Where, will this water flow? Will this water
flow into other waters? If so, describe.
All runoff from the onsite project and frontage improvements will be captured and
conveyed to underground infiltration systems to be infiltrated into the ground.
Roof runoff will be captured and directly conveyed to the onsite infiltration system.

Runoff from the parking lots will, be captured then conveyed to the onsite water
quality treatment facility to remove oil, sediment, and floatables then conveyed to
the onsite infiltration system.

It is not anticipated that waste materials will enter the ground or surface waters.

any:
The stormwater systems will be designed to the stormwater standards identified in
the Department of Ecology 1992 stormwater Management Manual for Western
Washington that are adopted by the City of Yelm. All storm runoff from onsite and
frontage improvements will be collected and infiltrated in the infiltration systems.
Water quality treatment will consist of StormFilter vaults and media cartridges
manufactured by Contech stormwater Solutions, Inc.
4. Plants

exception of the large Maple. Two Maples and a Horse Chestnut tree are within the
street right-of-way improvement width on Solberg and Yelm and will be removed
unless it is possible to divert the sidewalks around them as shown on the Site
Plan.

New vegetation will be installed in parking. islands and along property boundaries
per City of Yelm standards.

Electricity and natural gas are proposed for heating and lighting.

It is not expected that the project would affect adjacent properties' use of solar
energy.

Washington State energy requirements will be met when designing building shell,
lighting, heating, and ventilation equipment.
Environmental Health

There are no known environmental health hazards that will occur as a result of this

Current use of site is for a Dental office on the primary parcel and a single family
residence on the second parcel. Adjacent uses are moderate density residential to
the north and northeast, Institutional to the southeast, and small commercial to the "
northwest, west and southwest.

There will be nine (9) operatories available in the clinic and the staff to service the
patients has a potential total of 10-12 staff.

The proposal would displace the residents in the single family house.

Occupants of the single family residence have already sold the property to Today's
Dental owners and the house is temporarily occupied by a family member.

Both the Comprehensive Plan designation and the zone classification allows for
the development of retail/commercial projects such as proposed by this
development. Design of the development will conform to the City of Yelm Design
Standards and will be designed to be compatible with adjacent commercial
development.
Housing

No new housing is proposed.

Maximum building height is being considered at approximately 26'. Colors and
materials will be compatible and in alodge-style. Materials will include cedar or
hardi-plank beveled horizontal siding, hardi-panel siding, cultured stone cladding,
composition shingle main roof with metal panel feature roofs. Glazing will be
fiberglass windows or aluminum storefront.
b. What views in the immediate vicinity would be altered or obstructed?
The new building will sit in approximately the same location as the single story
residence and will not obstruct any views.

11. Light and Glare '.
~a. What type of light or glare will the proposal produce? What time of day would it mainly
occur?
Typical low-level parking lot lighting, which will be directed downward, will be
illuminated during early evening and morning business hours. Light from vehicle
headlights as cars navigate the parking area will be emitted from the site.
b. Could light or glare from the finished project be a safety hazard or interfere with views?
Not anticipated. There are no significant views that would be impacted.
c. What existing off-site sources of light or glare may affect your proposal?
None anticipated.
d. Proposed measures to reduce or control light and glare impacts, if any:
Light standards will be installed in locations that minimize the amount of light
encroachment on to adjacent properties.
12. Recreation

Tahoma Valley Golf Course, Yelm City Park !3< Cochrane Park are all within a'/z mile
radius of the project site.
b. Would the proposed project displace any existing recreational uses? If so, describe.
There is no existing recreational use on site.

Access to the proposed project will be from two driveway access points off
of Solberg Street. Sidewalks will be installed on the three street sides of the
property. Please see the attached Site Plan.
b. Is site currently served by public transit? By what means? If not, what plans exist for
transit service?
The site is currently along the Intercity Transit Route 94 with an outbound stop one
block to the north and inbound stop a couple blocks to the south. No changes are
anticipated.
c. How many parking spaces would the completed project have? How many would the
project eliminate?
Currently, there is an existing gravel parking area that can accommodate
approximately 11 vehicles. When the project is completed, a paved parking area
with a total of 23 parking spaces, including 2 handicapped accessible, would be
provided:
Will the proposal require any new sidewalks, trails, roads or streets, or improvements to
existing sidewalks, trails, roads or streets, not including driveways? If so, generally
describe (indicate whether public or private).
New right-of-way street improvements will be required on Yelm Avenue, Solberg
Street and Jefferson Street and will include road widening, sidewalk and landscape
areas.
e. Will the project use (or occur in the immediate vicinity of) water, rail, or air transportation?
If so, generally describe.
The proposed project does not require the use of, nor will it occur in the immediate
vicinity of water, rail or air transportation systems.
How many vehicular trips per day would be generated by the completed project? If
known, indicate when peak volumes would occur.
From the minutes of the Pre-Application Meeting, dated July 12, 2006, the Peak PM
Trip Rate is 3.06. The change in building footprint (proposed building at 4200 sf,
less the existing building at 1,980 sf) is approximately 2,220 sf.
3.06 x 2220 / 1,000 = 6.8 additional peak PM hour vehicular trips.

The proposal is an expansion of the existing on-site use and as such the need for
existing services to the project will continue. No increase in demand is
anticipated.
