2020.0282.RO0020 CR Scope of Work4 September 2020 The Prestige Senior Living – Rosemont Facility will be having the South facing side of the second story main building roof, re-roofed with new underlayment and shingles. OSB sheets will be replaced as necessary due to wear, tear, and water damage. The portion of the roof that is being replaced does not directly affect any residents' rooms as it is above the dining room, an office, and a portion of an activity room. All of which are not actively being utilized by any residents due to the state’s COVID restrictions. This project is expected to have little to no discernable impact on residents' everyday lives and routines. The biggest risks and impacts to the residents during this project that we have assessed are the noise, smell, dust, the dump trailer, and the material drop off vehicle. The roofing company who will be completing this project is Integral Roofing and Construction with a total estimated cost of $24,002 after tax. The estimated cost includes the removal and disposal of existing roof material, replacement of any worn or damaged OSB, material and installation of underlayment, shingles, vents, drip edge, pipe boots, skylights, valley metal and other flashings, as well as a 12-year warranty on the wo rk performed. The estimate also includes a safety officer on site and the installation of safety equipment and fall protection. Day one will consist of the removal and disposal of existing roof material and an assessment of the condition of the underlying OSB. The dump trailer provided by Integral will be staged to the rear of the facility near the garbage encloser. This area is not used by residents and should have little to no impact on their day to day life and routines. The dump trailer is expected to be in place for the duration of the project to consolidate all debris and waste. Caution tape and safety cones will be placed a minimum of three feet around the dump trailer to identify, deter and prevent residents from accessing this area. There are two resident accessible doors that will be affected by the project. These doors will be locked and caution taped off for the duration of the project to prevent any potential accidents or injuries to residents. These doors being locked is expected to have a very minimal impact on the resident's everyday life and routines. We will be going door to door to inform all residents of the upcoming repair project a minimum of 24-hours prior. For the residents on the project facing side of the building we will also recommend to them that they close their windows to mitigate the noise level and potential dust throughout the project. Day two will consist of all materials being delivered to the work site, replacement of any worn or damaged OSB found needing replacement during the day one assessment, and the installation of the underlayment, shingles, flashings, and boots. Integral is estimating completion of the project on day two, however, any work that is not able to be completed on day two will be done on day three. The primary plan for the material delivery will be to the rear of the facility so as not to disrupt the residents as much as possible. If the delivery vehicle is unable to get close enough to the building to unload the materials at the rear of the building our contingent plan is to use the front driveway of the building. Material drop off time is estimated to be no longer than 3 hours. In the event that the delivery has to take place in the front of the building, cones will be placed around the delivery vehicle to warn residents of safe distances from the vehicle. We will also assign one person as a ground guide as a look out and assist in guiding residents and vehicles around the delivery vehicle as needed. This event is expected to have the biggest impact on the residents due to the noise level, amount of moving equipment and large vehicle potentially in a high traffic area. We will make every effort to accommodate the residents and their families as best we can to prevent this event from affecting their day to day life, routines, and family visitations. All materials and tools will be staged on the second story flat roof to prevent any residents from accessing tools or materials. Overall, this project is projected to have a quick completion time and have very few impacts on our residents lives and routines. The point of contact for this letter is the undersigned. Keith A.
