Box 479 Yelm, WA 98597   June 11, 2008 Mike and Bev Malan PO Box 747 Yelm, WA 98597 RE: 16547 103rd Ave SE – City of Yelm Project No CIV-08-0210-YL Connecting Existing Duplex Residence to City Sewer Service Dear Mike and Bev: The City of Yelm received your request to connect to the City’s sanitary sewer system. We have reviewed your application and can approve this connection subject to the following conditions. Sanitary Sewer Requirements: The onsite sewer system must be decommissioned and a permit must be secured thru Thurston County for abandonment of the existing onsite sanitary sewer system. A new STEP sewer system must be installed by an approved septic installer and inspected by the City sewer department. The sewer service will require an open trench installation in 103rd Ave. Restoration of the trench area will consist of grinding a minimum of 10’ either side of the trench and overlaying this area with 2” asphalt concrete. The single tank installation will require a separate power meter for the operation of the tank and controller. This will require coordination with PSE and a separate Right of Way permit must be applied for by PSE. Documents Required for Approval: A step sewer utility easement (individual form) must be executed by the property owner. A step sewer bill of sale for the new step sewer tank and misc equipment. A waiver of protest must be executed by property owner for future annexation to the City. Fee Due for Connection: The sanitary sewer connection fee is $5,885.00 per a unit and $145.00 inspection fee per a unit. Total fees due are $12,060.00. All fees and documents must be executed and submitted before installation of the step sewer system may begin. I have attached the require documents and invoice for the fees required. Please return this information before you contact the sewer department for the first inspection.
