July 8 2003 Northwest Testing Company, Inc. CONSTRUCTION TESTING and INSPECTION ENVIRONMENTAL CONSULTATION and ASSESSMENT FIELD INSPECTION REPORT CLIENT:DATE: Mountain West Senior Housing LLC July 08/10/11, 2003 PROJECT:REPORT NO: Rosemont Retirement & Assisted Living 04 CONTRACTOR:PROJECT NUMBER: Maddox Construction 0306.9 PERMIT NO: INSPECTOR: 6957 Mark Robinson July 08: Inspector Mark Robinson on site, 9:30 am, for inspection of forms and reinforced concrete placement. Overcast, 68 degrees. Contractor places concrete for the elevator pit walls, and the exterior continuous foundation walls from grid 1 (H.1) south grid line 1 (E.1), and exterior continuous footings from grid line 4 (C.1) north, east, and south to grid line 5 (C.3). All areas approved for concrete per structural notes on sheet S1.0, plan notes schedules on sheet S1.1A, elevator detail 14/SD.1, shear wall anchor schedule and relevant details on sheet SD.1. Concrete is placed by truck chute at a 4.5” maximum slump and is vibrated for consolidation. One set of compressive strength test specimens is made on the placement of 9 yards and is stored in the field away from the work area for later pickup. See Report of Concrete Test Specimens for further details on today’s placement. Inspector expresses his concern regarding the floor slab placement scheduled for this Friday. The weather forecast is for a hot, sunny day, with concrete to be placed directly upon vapor barrier, a situation that often results in excessive cracking. The following specific items pertaining to the floor slab are discussed: 1)Slab is to be placed directly onto vapor barrier. Architectural detail 8/A3.2 shows 2” of sand over vapor barrier. However, the vapor barrier being used is specifically designed for direct concrete placement and has been approved. Inspector speaks directly with supplier on this point. 2)Contractor to properly cure slab with curing compound, and will water flood if necessary. 3)Intentional penetrations through the vapor barrier are to be avoided. Conventional grade pins penetrating the vapor barrier will not be allowed. Grade to be set by laser or other means. 4)Slab must maintain 4” designed thickness. Inspector recommends that sub-grade be thoroughly checked prior to placement of vapor barrier and reinforcing. Subgrade consists of 5/8” crushed rock, and is being water conditioned and compacted with a large diesel vibratory sled compactor. 5)Slumps in excess of 5” will not be allowed. Concrete supplier QC is contacted and further recommends that an additional ½ sack of cement be added to the mix to mitigate cracking. All these items are discussed among the general contractor, the sub-contractor responsible for the floor slab, and the concrete supplier. July 10: Inspector Mark Robinson on site, 9:15 am, for inspection of forms and reinforced concrete placement. Sunny, 74 degrees. Contractor places concrete for the exterior footings at grid lines 5 (C.3) east to 8.2 (C.3) and the exterior foundation walls line 1 (E) south and east to 4.2 (E). All areas approved for concrete per structural notes on sheet S1.0, plan notes schedules on sheet S1.1A, and the shear wall anchor schedule and relevant details on sheet SD.1. Concrete is placed by truck chute at a 4.5” maximum slump and is vibrated for consolidation. One set of compressive strength test specimens is made on the placement of 15 yards and is stored in the field away from the work area for pickup tomorrow. See Report of Concrete Test Specimens for further details on today’s placement. Inspector reviews the work completed so far in preparation for tomorrow’s floor slab placement. Inspector notes to contractor that the architectural detail 8/A3.2 indicates rigid wall insulation on all exterior walls (not shown on structural wall detail). Slab is being constructed without insulation. Inspector requests that contractor clarify this with architect (see below notes). July 11: Inspector Mark Robinson on site, 6:30 am, for inspection of forms and reinforced concrete placement. Sunny to overcast to sunny, 66 degrees. Contractor places concrete for the floor slab from grid line 10 east. Graded at 4” uniform depth with insulation at walls per detail. #3 reinforcing bar in place at 18” on center each way. Thickened wall footings and pads in place and reinforced per relevant details on sheet SD.1. Vapor barrier in place with all edges and mechanical penetrations taped and sealed. Placement without rigid insulation at perimeter wall is approved, provided vapor barrier is run up the inside edge of the foundation wall, as is the case. All subgrade conditions have been well prepared. Concrete is placed by pump at a 5” maximum slump and is vibrated for consolidation in thickened wall footing areas. Three sets of compressive strength test specimens are made on the placement of 135 yards and are stored in the field away from the work area for later pickup. See Report of Concrete Test Specimens for further details on today’s placement. NORTHWEST TESTING COMPANY, INC.
