PROJECT:REPORT NO:

CONTRACTOR:PROJECT NUMBER:
Maddox Construction 0306.9
PERMIT NO: INSPECTOR:
6957 Mark Robinson
July 29: Inspector Mark Robinson on site, 1:00 pm, for review of epoxied anchor installation and
reinforced concrete placement. Clear, 76 degrees.
Anchor Bolt Installation Procedure:
Materials: Hilti HAS HF 7/8” x 16”; Hilti HAS E 58 x 17” anchor bolts
Hilti HY-150 epoxy
Inspector reviews structural notes and details 2 and 3/S0.00. Locations completed today include all
interior wall anchor bolts east of grid line 10. (Note: two locations were missed at grid line 12 (G.2)
and 10 (G.2). Contractor informed and will insert anchors at next inspection).
The holes for reinforcing have been drilled with the diameter of the drilled holes exceeding the
reinforcing rod diameter by approximately 1/8”. All holes are blown free of debris and dust, brushed
and re-blown out with pressurized air such that no dust or debris is noted leaving hole when
compressed air is introduced.
After ensuring proper color (mixing) of epoxy at the end of the dispenser nozzle the holes are filled
approximately 2/3 full with epoxy, and the reinforcing bar are inserted into the hole with a slight
clockwise rotation, with a 1/8” counter-clockwise turn upon complete penetration. Rods are randomly
checked to ensure complete epoxy coverage.
Contractor places concrete for the exterior footings and foundation wall (monolithic placement),
grid line 10 (D-D.2), D (8-10). All areas approved for concrete per structural notes on sheet S1.0,
plan notes schedules on sheet S1.1A, Steel form spreaders used.
Concrete is placed by truck chute at a 4.5” maximum slump and is vibrated for consolidation. One
set of compressive strength test specimens is made on the placement of 5 yards and is stored in
the field away from the work area for pickup tomorrow. See Report of Concrete Test Specimens
for further details on today’s placement.
July 31: Inspector Mark Robinson on site, 7:50 am, for inspection of forms and reinforced concrete
placement. High overcast, to sunny, 61 degrees.
Contractor places concrete for the floor slab between grid line 4.2 and 10. Graded at 4” uniform
depth. The #3 reinforcing bar is in place at 18” on center each way and supported mid-way in
slab. Thickened wall footings and pads in place, sized and reinforced per schedules on sheet
S1.1A. Vapor barrier in place with all edges and mechanical penetrations taped and sealed, and
placed to envelope foundation wall. All subgrade conditions are well prepared.
Concrete is placed by pump at a 5” maximum slump and is vibrated for consolidation in thickened
wall footing areas. Three sets of compressive strength test specimens are made on the placement
of 111 yards and are stored in the field away from the work area for later pickup. See Report of
Concrete Test Specimens for further details on today’s placement.
