CONSTRUCTION TESTING and INSPECTION ENVIRONMENTAL CONSULTATION and ASSESSMENT FIELD INSPECTION REPORT CLIENT:DATE: Mountain West Senior Housing LLC June 19, 2003 PROJECT:REPORT NO: Rosemont Retirement & Assisted Living 01 CONTRACTOR:PROJECT NUMBER: Maddox Construction 0306.9 PERMIT NO: INSPECTOR: 6957 Mark Robinson Inspector Mark Robinson on site, 8:45 am, for inspection of forms and reinforced concrete placement. High overcast, 60 degrees. Contractor places concrete for the exterior continuous building footings south of grid line 10.6 (no column pads placed). Inspector meets with project superintendent and obtains a set of building plans. Inspector reviews mix design and structural notes on sheet S1.0, plan notes and schedules on sheet S1.1B, and relevant details on sheet 3SD.1. Inspector notes that the sub-contractor (Finishing Edge) is reinforcing the holdowns locations with two vertical #4 hooked bars per detail 3/SD1. Today the 24” length holdowns have to be inserted into the footings (mark numbers 3 and 4). Inspector notes one location where holdown is missing. This is corrected prior to concrete placement. Concrete is placed by truck chute at a 4.5” maximum slump and is vibrated for consolidation. One set of compressive strength test specimens is made on the placement of 22 yards and is stored in the field away from the work area for pickup tomorrow.
