Twin Star Special Insp Northwest Testing Company, Inc. CONSTRUCTION TESTING and INSPECTION ENVIRONMENTAL CONSULTATION and ASSESSMENT FIELD INSPECTION REPORT CLIENT:DATE: Twin Star Credit Union April 03, 2008 PROJECT:REPORT NO: Yelm Branch Addition 01 CONTRACTOR: PROJECT NUMBER: Schwiesow Construction 0803.6 PERMIT NUMBER:INSPECTOR: BLD 070419 YL Mark Robinson Inspector Mark Robinson arrives on site, 12:45 pm, for project overview with contractor and for inspection of subgrade compaction. Sunny, 52 degrees. Contractor has completed the excavation and yield-compaction for the four canopy footing pads. The exposed native soil consists of loamy silt with gravel (SM). The subgrade has been yield-compacted with the hoe-pak. While the earth appears well consolidated, inspector does note that the soil appears to be below optimum moisture content for this soil type. Two in-place field density compaction tests are made at randomly chosen footing locations, and a sample of the soil is taken for moisture-density testing. Subsequent laboratory results indicate the field tests fall below the 95% compaction requirement. Discussion: as was noted in Bradley-Noble’s geotechnical report for the project, it is difficult to moisture-condition this soil type. Any water added has to be given adequate time to percolate evenly throughout the soil prior to compaction efforts, otherwise, the soil liquefies beneath the compactor. This makes it difficult to work the soils in inclement weather. Rain over the weekend will have percolated through the soil and should bring the moisture content close enough to the optimum to attain the 95% compaction requirement. If the rain stops, the contractor is given the option of trying to mechanically re-compact the subgrade or to over- excavate 12” and bring in compacted crushed rock or other suitable gravel that is less moisture- sensitive. Crushed rock is recommended since it will be required later beneath asphalt areas, thus eliminating additional laboratory proctor expenses later on. NORTHWEST TESTING COMPANY, INC. Showers, 47 degrees. Inspector meets with earthwork contractor’s superintendent Donnie on site for re-compaction of the canopy footing subgrade areas. Rain over the weekend has markedly improved the moisture content of the subgrade soil. Water has percolated through and there is no standing water in the footing areas. All four footing pads are re-compacted with the hoe-pak. Previously failing areas are re-tested and now found in compliance with 95% minimum compaction requirements. See attached for test locations and results. NORTHWEST TESTING COMPANY, INC. Partly sunny, 75 degrees Contractor drills and epoxies #4 reinforcing dowels into the side of the existing footing and foundation walls to connect to the addition footing and foundation walls. Three #4 bar are placed in the footings, two #4 bar mid-way in the wall, and two #4 bar across the top of the wall (total of 7 bars per footing/wall connection). Materials: #4 x 36” dowel bars; Hilti-150 epoxy, expiration date 12/2008. Procedure: The holes for the dowels have been drilled with a minimum embed depth of 8”. All holes are then prepared with each drilled location blown out, brushed and re-blown with 100-psi pressurized air through a tube extending to the back of the drilled hole until no further dust or debris leaves hole. Dowels are placed with a bleed-out of epoxy upon completion to ensure full encasement of dowel. Contractor has scheduled concrete placement for the addition footings early tomorrow morning. Inspector reviews plan notes on sheet S2.1 and typical wall and column footing details on S4.1. Upon inspection, inspector notes that the details specify #5 bar throughout the footings and foundation walls, both horizontal and vertical bars. Contractor has placed #4 bars in all areas except for #5 bar has been used in the column footing mats. Inspector informs contractor, concrete placement is cancelled. Field Note: As noted above, #4 dowel bars have been epoxied into the existing footing and foundation wall. These bars also should have been #5 bars. Contractor will have to get approval from the engineer accepting this bar, or alternatively, can add additional #4 bar at these locations. NORTHWEST TESTING COMPANY, INC. Partly cloudy, 52 degrees. The contractor has completed the assembly of the two exterior tube steel columns to beam connections for the entry canopy area. The “turn of the nut” method is used for tensioning of bolted assemblies. ASTM A-325 ¾” x 2 5/8” bolts with hardened washers have been used. Fasteners are noted to be free of rust and grime and retaining their original factory lubricated coating. All connections are brought into a snug tight condition with an ordinary spud wrench, working from any spaced areas between the column plate and the beam plate toward firm fit areas. The nuts (turning element) and back plates are then marked with a black marker. Holding the bolt head stationary, the nuts are then turned one-third revolution to achieve proper tension. All fasteners are addressed. NORTHWEST TESTING COMPANY, INC. Cloudy, 47 degrees. Contractor had intended to check the tube steel column to I-beam connections at the exterior of the vestibule, but does not have the correct wrenches on site. The tube steel column to concrete connection (exterior wall north of greeter room) is checked for bolt tension. These connections are not considered high strength connections. Connection includes ¾” anchor bolts (two are exposed). Brought to snug tight connection and turned additional one half turn. Inspector will be re-scheduled for the ASTM 325 bolt connections. Cloudy, 57 degrees. The contractor has completed the assembly of the tube steel columns to beam connections for the drive-through canopy area. Contractor has used the A325 – ¾” Ø x 2 ¼” high strength bolts with hardened washer per the project plans. “Turn of the nut” method to be used for tightening of bolted assemblies. Fasteners are noted to be free of rust and grime and retaining their original factory lubricated coating. At this time the connections have already been tensioned with an impact wrench. Inspector loosens several connections to determine if the bolts have not been over-tensioned. It is found that the connections loosen after a one-sixth back turn of the nut (one nut flat), so the bolts are not over-tensioned. All connections are then loosened and the nuts are brought into a snug tight condition with an ordinary spud wrench, working from any spaced areas between the column plate and the beam plate toward firm fit areas. The nuts (turning element) and back plates are then marked with a chalk. Holding the bolt head stationary, the nuts are then turned one-third revolution to achieve proper tension.
