RE: CONDITIONS OF APPROVAL SUMMARY FOR "McKENZIE MEADOWS"

Dear Ms. Tami:
As part of the final plat process for the McKENZIE MEADOWS PLAT (Cullens Road Plat), a review
of the conditions of approval was completed. A copy of the condition is attached for your
convenience. The conditions were met as follows:
1. The conditions of the MDNS have been met as described in the hearing examiner's conditions
of approval.
2. The 4" diameter sewer main has been installed as shown on the "as-built" drawings. Each
dwelling unit in the subdivision will be connected to the City water system. The connection fee
and inspection fee will be paid at the time of building permit issuance.
3. The 10" diameter water main has been installed as shown on the "as-built" drawings. Each
dwelling unit in the subdivision will be connected to the City water system. The connection fee
and meter fee shall be paid at the time of building permit issuance. All conditions for cross
connection control have been met.
4. Documentation of a protective well covenant has been provided with the final plat submittal.
5. The TRANSPORTATION FACILITY CHARGE will be paid by the client at the time of building
permit issuance.
6. The roadways associated with this project have been constructed to the City's improvement
standards. The East travel lane of Cullens Road has been constructed to provide a minimum
of 12 feet of width as verified in the field.
7. The FIRE PROTECTION IMPACT FEE will be paid at time of building permit issuance.
8. OLYMPIC REGION CLEAN AIR AUTHORITY (ORCAA) approval was received prior to
demolition of the existing mobile home.
9. A total of 0.71 acres has been provided as qualified open space,

11. The illumination design has been approved by the City of Yelm.
12. An addressing map has been submitted for review and approval.
13. The subdivision name of "McKenzie Meadows" has been reserved with Thurston County. A
"Plat Name Reservation Certificate" is included in the final plat submittal.
14. A landscape and irrigation plan has been submitted to the City of Yelm and was constructed as
part of the project.
15. 100 trees (deciduous and evergreen) have been planted for mitigation of trees that cannot be
preserved. All trees have been planted in the primary tree tract, secondary tree tract, and the
open space areas. Please see the approved landscaping plans.
16. A maintenance bond for landscaping has been provided with the final plat submittal.
17. The civil engineering plans have been submitted and approved by the City of Yelm.
18. An approved fire hydrant plan is included in the civil engineering plans for this project.
