Dear Mr. Polaeios-Montes,
We received your application and plans for the above-referenced food establishment to be
located at 305 Yelm Ave E, Yelm, WA. Based on the information provided with the
application and subsequent e-mail communications, we are able to move forward and
approve the plans for the Twister ponuts II. Although we have approved your plans, there
are several expectations and conditions that shall be addressed prior to scheduling a pre-
opening inspection. Please address each of the following items accordingly and see that
they meet our requirements:
Specific to Your Plans
1. The three-compartment sink must be indirectiv drained. This means that there
must be a gap in the drain pipe before it connects to the sewer line. See the included
example page.
2. Since your donut display case is not refrigerated, you will not be able to sell donuts
that require refrigeration, such as the breakfast donuts with ham and cheese.
General Reauirements
1. You must pay for your annual food establishment permit and any accrued plan
review/pre-opening inspection fees before you will be given permission to open your
establishment. Allow at least one business day after your final pre-opening
inspection to finalize your bill. You may make payment in person, over the phone
with a debit/credit card, or send a check through the mail. Payments must be
received by this office before approval will be given to open your facility.
2. All openings around utilities and conduit passing through the walls and floors must
be sealed.
3. All building, planning, fire and electrical code requirements must be completed
prior to calling for a pre-opening inspection.

4. All hand-wash sinks must be provided with soap and paper towels (and dispensers).
Hot and cold water, under pressure, must be available at all times at hand-wash
sinks and three-compartment sinks.
5. Our records indicate that your establishment is of the following billing category
with the corresponding square footage. If our records are incorrect, please let us
know.

When the above issues have been addressed, you may contact us to schedule a pre-opening
inspection. All construction and construction debris must be removed, and all food contact
surfaces cleaned and sanitized before pre-opening inspection. Make sure the refrigeration
units are on, cold, have thermometers and a container of water for temperature testing.
You will also need a food probe thermometer, sanitizing buckets and test strips
