Use this form if you are proposing a connection to a community or other public water system
Purpose. To assure the availability of potable drinking water prior to the issuance of a
building site approval or building permit to meet the requirements of Chapter
19 27 RCW, WAC 246-290, and the Thurston County Sanitary Code, Article 3
Applicabilitv. Completion of this form and issuance of a Certificate of Water Availability is
required for all building site applications, residential site plans, or building
permits for new construction or change of use when potable water is required

CERTIFICATE OF WATER AVAilABiliTY
CONNECTION TO A PUBLIC WA TERSYSTEM
This form IS to be used to demonstrate that an approved water source is available
pri6r to the issuance of building site approval or a building permit when the proposed
structure(s) needs water The applicant must have the form completed by the owner
or operator of the public water system and submit the form with the Building Site
Applicat:on or Residential Site Plan
Thurston County Environmental Health staff will evaluate the water supply records for
the public water supply to assure that the water system has approved construction
plans, proposed connection is WithIn the scope of the system approval, and the water
system IS In compliance with water quality monitoring and other operational
requirements
The review process is as follows:
STEP 1
STEP 2
STEP 3
STEP 4
STEP 5
Have the middle section of the form completed by the owner or operator
of the public water system
Submit the form with the Building Site Application (BSA) or Residential
Site Plan (RSP)
Environmental Health staff will review the water system records for the
water system concurrentlv with the other reviews of the BSA or RSP
You will be notified at the earliest possible time if records indicate
problems With the water system and if the Certificate of Water
Availability can not be issued You rnay want to discuss this issue with
the water~ purveyor at the tIme you complete STEP 1 The water
purveyor should know if the system is approved for further connections
and is in compliance with operational requirements.
Contact the public water system purveyor ImmedIately If you are notified
of a problem Some problems may be resolved quickly to allow the
issuance of permits (eg the system is late in submission of the latest
water sample) while others may take more time (eg the water system
does not have approval or capacity for more connections).
Environmental Health staff will approve the Certificate of Water
Availability which demonstrates water is available.

That we the undersigned having a real Interest In the tract of land
described by the declaration; and do hereby declare the herein described
adjustment of land certified as Boundary Line Adjustment Number BLA- 8118
on the 17th day of June , 19 94 , by the Planning
Department, subject to the following covenants and conditions
1. That all subsequent deeds will contain provisions for private
roads in the manner described herein.
2. That all maintenance of any private road described by this
declaration shall be by the owners of the parcels having legal access
therefrom or their heirs, assigns, or successors, unless and until such roads
drc improved to the subdivision standards and dedicated to and accepted by
the appropriate governmental Jurisdiction.
3. That any private road will be subject to the further right of
the grantor or his successor and of any telephone, electric, gas, watcr, or
sewer company, public or private, to lay or cause to be laid and the right of
Ir1gress or egress for the purpose of maintaining telephone, electric, gas,
water or sewer pipes, mainS,. or conduits across a described portion of such
rO<.ld.
4. That with respect to any private road described by th is
declaration whether it remains private or becomes a dedicated road, tnere is
the additIonal right to make all necessary slopes for cuts and fills, Jnd the
fights to continue to drain said roads and ways over and across any lot or
lots where thc water might take a natural course upon reasonable grading
pursuant to improvement for dedication of the roads and ways shown herein.
FollOWing reasonable grading pursuant to improvement for dedication of the
roads and ways shown herein, no drainage waters on any lot or lots shall be
diverted or blocked from their natural course so as to discharge upon any
publ ic road rights-of-way or to hamper proper road drainage,
5. That the adjusted legal descr iption of each of the tracts being
adjusted IS attached hereto and incorporated by reference ;)s though fully set
out herein.
6. That additional covenant, easements, restrictions, if any, solely
for the benefit of the grantor, and his heirs, successors, and assigns

. The closure calculations which I have enclosed indicate an error both on the individual
parcels and on the total boundary If I am correct, the legal description will also require
revision.
. The type of survey equipment used needs to be identified on the BLA sheet.
. The title block which reads City of Yelm Short Plat should read City of Yelm Boundary
Lrne Adjustment.
. The statement concerning roads and access on the short plat border is not appropriate
for a BLA.
