Wood, MD, MPH Health Officer RE: INITIAL WALK -THRU INSPECTION RESULTS FOR MARIACHI ALEGRE Dear Mr. Rodriguez: At the request of your contractor Rick Christensen, the health department arrived for an initial walk -thru inspection for the above - referenced food establishment located at 16605 106'h Ave SE, Yelm, WA. Based on the inspection (which lasted 1.5 hours), we found several issues concerning the construction, finishing touches, lighting, changes in approved plans, insufficient storage, design flow and several plumbing problems. Prior to moving forward with any further inspections, the following items shall be addressed: 1. The approval letter dated March 10, 2016 required additional information to be addressed PRIOR to a pre- opening inspection. We have not received a response to this letter that was addressed to you. As the owner of this food establishment, it is your responsibility to respond to this letter. 2. The storage inside the kitchen area is non - existing. Need to provide and identify storage for receiving soiled/dirty dishware, pots /pans, drying equipment, clean equipment, chemical food storage and dry food storage. Per discussion with your contractor Rick Christensen, we are requiring a part of the three compartment sink to accept dirty dishes by providing a washable white acrylic cutting board over the left side drain board and left compartment. Also, we are requiring a 36" shelving for drying equipment/utensils to the very left of the hand sink at the end of the cook's line. The dry food storage will require a 60" shelving adjacent to the soda bag rack and a 30" shelving directly across next to the water heater. Still need to identify the location for clean (already dried) equipment, dishware and utensils. Recommend magnetic strips for utensils on the cook's line. We assume chemical storage will be in the janitorial closet. Provide location for single- service items (i.e. to go containers). Please confirm all the above storage locations. 3. Poor lighting directly over the waitress station and the bar equipment. Track lighting will be provided per the contractor and shall illuminate these area. Also, provide under counter lighting beneath the bar. 4. Provide an additional light fixture (i.e. can light) directly above the food sink as discussed with the contractor. 5. The bar hand sink shall be relocated since it is not easily accessible. Discussed already with plumber. 6. The exposed wooden platforms and wood panel walls are not acceptable for the equipment behind the bar for the keg refrigerator. Absolutely no exposed wood allowed. The finish schedule shall be protected from absorption and allows the surface to be easily clean. Per the contractor, he will line the entire inside with FRP and provide trim as necessary. 412 Lilly Rd. There is a floor sink that is clearly not visible or accessible beneath the keg refrigerator. Floor sinks are considered pieces of equipment and should be easily accessible for cleaning and maintenance. In addition, the p -trap will dry if not heavily used. According to the City of Yelm Building Department, a retrofit trap primer will be required to be installed with this floor sink since there is no constant water flow to this particular floor sink. Provide proof (i.e. picture and invoice) this device was installed. With the addition of the trap primer, we will allow the current location for this floor sink on a performance basis. If there is any reason to believe this plumbing fixture is causing plumbing or sanitation problem, it will be made permanently accessible. 8. Caulk around the mop sink basin. 9. Exposed wood trim (plywood) directly beneath the pass -thru shelf. Also, exposed wood trim (i.e. plywood) along the entire bar counter underneath the concrete. Provide material that is washable, durable, and nonabsorbent. 10. Provide approved shelving inside the walk -in cooler. Provide wire shelving to provide air circulation. 11. The backflow to the CO2 shall be secured, plumbed to an indirect drain and tested. The current location is not acceptable due to the likelihood of it getting damaged. Per the contractor, the device will be raised, secured and plumbed to the drip pan for the water heater. Have the device tested. This space is then available for additional dry storage shelving. 12. The soda lines located partly on the floor behind the bar and installed make for difficult cleaning. Keep the lines completely off the ground and wrap the lines with a durable material to allow easy and routine cleaning of these lines. 13. Provide method to dry glasses and stock glasses in the racks behind the bar. There is a piece of bar equipment designed for this purpose and is situated directly next to the dishwasher. Provide. 14. To avoid spillage of drinks and creating unsanitary conditions, install a spill tray along the bar when filling drink orders and have it drained to the floor sink below. Do not allow the drain pipe to suspend directly over the open ice. 15. The tap for the bar does not have a spill tray to avoid spill and creating unsanitary conditions. Provide a receptacle or spill tray to accept overflows or spill and allow it to drain in an approved manner. Since the floor sink is covered and not accessible for periodic cleaning and maintenance, provide an approved and sanitary method to accept the spillage from the spill tray. 16. Close or seal gap with trim along the back of the bar counter where it connects to the wall. 17. The decorative wood shelving along the back wall of the bar are raw wood. This is acceptable, in this case, but the wood should still be cleanable and can easily be dusted and sealed. 18. Provide hand wash signs for all hand sinks (including those in the restrooms) 19. The large white shelving boards located behind the waitress station in the dining area is new to the design. These boards are compressed wood and not designed nor built to withstand the constant use for commercial application. Please provide alternative shelving approved for this use and storage that is durable. Recommend the use of stainless steel table(s) for this purpose. In fact, since a food preparation cooler is provided in this area, at least one approved stainless steel table needs to be provided and adjacent for food preparation cooler. 20. The steamtable choice of equipment requires the steam well to drain for routine cleaning and maintenance. A floor sink is not available. Provide an approved and sanitary method to accept the water draining from the steamtable wells. 21. The latest plan suggest by the contractor in March called for a hot holding cabinet and it was to be located left of the hand sink at the end of the cook's line. However, due to lack of shelving for drying equipment as a result of missed calculations with the design, there is no other location to provide this necessary piece of equipment for properly drying all pots, pans, dishware and utensils. As it stands, there is no other location to store the hot holding cabinet. 22. Provide magnetic strips to store clean utensils, such as knives, and make them accessible for use. 23. All refrigeration units shall be available and operating correctly at 41'F or less. Please verify. 24. Due to the number of changes to the food establishment (including the bar), please provide an updated and accurate scaled layout of the entire facility with ALL equipment shown as installed. 25. Provide storage area for employees to store personal items and NOT inside the kitchen/bar area. 412 Lilly Rd. Additional corrections or changes are common as the completion of this food establishment progresses. However, we will not schedule a pre- opening inspection until we receive a written response to each of the items listed above and approved. As a reminder, this department still needs a written response to our original plan review approval letter dated March 10, 2016. Without this information in response to the approval letter and all other corrections noted in this letter, there is nothing else we can do or schedule to complete this food establishment for opening.
