follows:

Observed the placement of ASTM A615,grade 60 reinforcing steel into 1'2"&1'5"Stem walls and 3'6"x11"spread footings mark F3.5 on
8/S3.10 at the above mentioned locations.
Reinforcing steel was of proper size,grade,lap,spacing,location,clearance,and secure prior to placement of concrete.
Items noted to contractor for correction prior concrete placement:
1.Anchor rods hold down plate washer requires minimum clearance of edge form,contractor made corrections no further action required.
2.Per detail 2/S4.11 Contractor requires horizontal reinforcing each side of vertical pipe penetration,contractor added reinforcing per
approved plan no further action required.
Embeds:CB TYPE base by a Simpson Strong tie installed per 9/S3.10
Anchor bolts:Per detail 5/S4.11 Contractor installed 5/8"x12"all thread rods with(2)1/2"x3"x3"plate washers and double nut installed per
plan.All plate anchorage of 5/8"diam.Bolts at 48"oc perschedule on 5/S4.10 and 1/S4.11 not installed at time of inspection,contractor to
"wet set"into place please verify at concrete placement.
Work performed,that was visually inspected was found to be in conformance with project plans dated 10-23-14 rev. 1.
