PO Box 808 Montesano,WA 98563 ASBESTOS SURVEY AAA Abatement and Demolition PO Box 808, Montesano,WA 98563 Property located at: 115 Mosman SE,Yelm,WA(Former Police Station) Owner: City of Yelm Owner's Representative and Number: Gary Carlson, City of Yelm Building Official,360-458-8407 On January 23,2014,AAA Abatement personnel conducted an Asbestos Survey(per U.S.E.P.A/A.H.E.R.A. guidelines as designated and specified by Olympic Region Clean Air Agency and Washington State) of the property located at 115 Mosman SE, Yelm, WA. This survey purpose is to identify any Asbestos Contained Materials that may be present and will require professional removal prior to renovation or demolition. All visible and accessible suspect material was sampled. If during demolition, hidden suspect material is uncovered, it should be presumed ACM and demolition should cease until further sampling can be conducted. NARRATIVE OF FINDINGS This approximate 3300 square foot one story, commercial building was built with T1-11 siding and had vinyl windows. There was an asphaltic torch down roof and a composition parapet roofing material existing, as well. Building had baseboard heat and an HVAC heating system-there was no TSI observed. Some of the wallboard/joint compound had wood paneling; however, all the wood paneling was nailed —not glued. Two different colored cove bases existed and both were sampled. All SVF and carpet glue were sampled throughout since there were a few different variations. Fiberglass insulation was observed and there was nothing suspect in the attic crawl space area. The age is unknown. Many fluorescent bulbs existed throughout the entire building. There was a drop tile ceiling with the top layer of ceiling tile being stapled. The property was in average condition. The property is slated for a demolition. Photos are available upon request with authorization to release by Owner. Montesano, WA 98563 RE: Bulk Asbestos Fiber Analysis, NVL Batch#1401437.00 Dear Ms. Bongimo, Enclosed please find test results for the bulk samples submitted to our laboratory for analysis. Examination of these samples was conducted for the presence of identifiable asbestos fibers using polarized light microscopy (PLM) with dispersion staining in accordance with both U.S. EPA 600/M4-82-020, Interim Method for Determination of Asbestos in Bulk Insulation Samples, as found in 40 CFR, Part 763, Subpart E,Appendix E (formerly Subpart F,Appendix A),and U.S. EPA 600/R-93/116(July 1993)Test Methods. For samples containing more than one separable layer of materials, the report will include findings for each layer (labeled Layer 1 and Layer 2, etc. for each individual layer). The asbestos concentration in the sample is determined by visual estimation. For those samples with asbestos concentrations between 1 and 10 percent based on visual estimation, the EPA recommends a procedure known as point counting (NESHAPS, 40 CFR Part 61). Point counting is a statistically more accurate means of quantification for samples with low concentrations of asbestos. if you would like us to further refine the concentration estimates of asbestos in these samples using point counting, please let me know. This report is considered highly confidential and will not be released without your approval. Samples are archived for two weeks following analysis. Samples that are not retrieved by the client are discarded after two weeks. Thank you for using our laboratory services. Please do not hesitate to call if there is anything further we can assist you with.
