the 5-Corners signalization project. His financial participation in the sIgnalization
project is to be credited toward hiS SEPA MitIgation fee for the development of Lot 1,
Creek Place Business Park. The Lot 1 mitigation fee of $22,200 00 is credited
$15,30507 resulting in an adjusted mitigation fee of $6,894 93 Attached are the

comments:
1) We request that utility easements be included in the
ingress egress easements.
2) Yelm Telephone Company will provide service to the
project in accordance to our tariffs filed with the
Public Utilities Commission.
3) We also request that the developer schedule meetings on
site with us during the design and construction phases for
the purpose of cordinating the location of buried Fiber
Optilc cables.
