as proposed by Fred Murray We revIewed the enVIronmental checklIst and have the followmg comments
ErosIOn control measures must be m place pnor to any cleanng, gradmg, or constructIOn. These control measures
must be effectIve to prevent soil from bemg carned mto surface water by stormwater runoff. Sand, silt, and soil will
damage aquatIC habitat and are conSIdered pollutants
Any dIscharge of sedIment-laden runoff or other pollutants to waters of the state IS m VIolatIOn of Chapter 90 48,
Water Pollution Control, and WAC 173-201A, Water QualIty Standards for Surface Waters of the State of
Washmgton, and IS subjectto! epforcementactIOn. ,\ i ,
Proper dIsposal of constructIOn debns must be on land m such a manner that debns cannot enter the natural
stormwater dramage system or cause water qualIty degradatIOn of state waters
Proper eroSIOn and sedIment control practIces must be used on the constructIOn SIte and adjacent areas to prevent
upland sedIments from entenng the natural stormwater dramage system. All areas dIsturbed or newly created by
constructIOn actIVItIes must be revegetated, use bIOengmeenng technIques, use clean durable nprap, or some other
eqUIvalent type of protectIOn agamst erOSIOn when other measures are not practIcal.
After completIOn of thIs project, there IS a likelihood that stormwater runoff will contam mcreased levels of grease,
oils, sedIment, and other debns. It IS recommended that stormwater treatment deVIces be mstalled so that any
dIscharge will be appropnately treated to remove these substances.
Routme mspectlOn and m~mtenance of all sedIment and erOSlOn control deVIces IS recommended both dunng and after
development of the SIte
A sedImentatIOn and eroSIOn control plan for the project SIte may be reqUIred and must be developed by a lIcensed
engmeer The plan must be Implemented before and dunng constructIOn. The SIte must be carefully momtored to
determme effectIveness of the plan. The plan must be upgraded as necessary dunng the constructIOn penod.
Dunng constructIOn, all releases of oils, hydraulIc flUIds, fuels, other petroleum products, pamts, solvents, and other
deletenous matenals must be con tamed and removed m a manner that will prevent theIr dIscharge to waters and soils
of the state The cleanup of spills should take precedence over other work on the SIte.
For All Proposed Sewered Developments:
Engmeenng reports, plans and specIficatIOns must be submItted to the Department of Ecology for approval. Reports

Thank you for the opportunity to comment on the detennination of nonsignificance for the constmction of a new
26,532 square foot assisted living facility for 48 residents (Easthaven Villa), located at 311 Cullins Road Northwest
as proposed by .Fred Murray We reviewed the cnviromnental checklist and have the following comments.
Erosion control measures must be in place prior to any clearing, grading, or constructIOn. These control measures
m~lst be effective to 'prevent soil from being carried into surface water by stonnwater mnoff. Sand, s11t, and soil w1l1
damage aquatic hab1tat and are considered pollutants.
Any discharge of sediment. laden runoff or other pollutants to waters of tho state is in violation of Chapter 90 48,
Water Pollution Control, and WAC 173-201A. Water Quality Standards for Surfhce Waters of the State of
Washington, and is subject to enforcement action.
Proper disposal of constn~ction debris must be on land in such a manner that debns cannot enter the natural
stormwOltor drainngo system or cause water quality degradation of state waters.
Proper erosion and sediment control practices must be used on the construction site and adjacent areas to prevent
upland sediments from entering the natural stormwater drainage system. All areas disturbed or newly created by
constmction actIvities must be revcgetated, uso bioengineering techniquos, use clean durable nprap, or some other
equivalent type of protection against erOSlOn when other measures are not practical.
After completion of this project, there is a likelihood that stonnwater nmoff wIll contain Increased levels of grease.

discharge will he appropriately treated to remove these substances.
Routine inspection and maintonance of all sediment and erosion control devices is reconunended both during and after
development of the Site.
A sedimentation and erosion control plan for the project slte may bo required and must be developed by a licensed
engineer The plan must be implemented before and during constnIction. The site must be carefully momtorcd to
detennme effectiveness of the plan. Tho plan must be upgraded as necessary during the constmction period.

ENVIRONMENTAL CHECKLIST
Instructions:
The State Environmental Policy Act (SEPA) requires all governmental
agencies to consider the environmental impacts of a proposal before
making decisions. Th~ purpose of this checklist is to provide
information to help identify impacts from your proposal, to reduce
or avoid impacts from the proposal if it can be done,."and to help
the City decide whether an EIS is required. An environmental
impact statement (EIS) must be prepared for any proposal with
probable significant adverse impacts on environmental quality.
Th.i.s.. environmental checklist asks you to describe some basic
information about your proposal. The City will use this checklist
to determine whether the environmental impacts of your proposal are
significant and require preparation of an EIS. You must answer
each question accurately, carefully and to the best of your
knowledge. Answer the questions briefly, but give the best
description you can. In most cases, you should be able to answer
the questions from your own observations or project plans without
the need for experts. If you do not know the answer, or if a
question does not apply to your proposal, write "do not know" or
"does not apply". Complete answers to the questions now may avoid
delays later. If the space provided is too small, feel free to
attach additional sheets.
Some questions ask about governmental regulations, such as zoning,
shoreline, and landmark designations. Answer these questions if
you €an. If you have problems, the city staff can assist you.
The checklist questions apply to all parts of your proposal even if
you plan to do them over a period of time or on different parcels
of land. Attach any additional information that will help describe
your proposal or its environmental effects. You may be asked to
explain your answers or provide additional information for
determining if there may be significant adverse impacts.
Nonproject Proposals Only:
Complete both the checklist (even though many questions may be
answered "does not apply") and the Supplemental Sheet for
Nonproject Actions (part D). For nonproject actions, the references
in the ckecklist to the words "project," "applicant," and "property
