The State Environmental Policy Act (SEPA) requires all
governmental agencies to conSider the environmental impacts
of a proposal before making decisions The purpose of this
checklist is to proVide information to help identify impacts from
your proposal, to reduce or avoid Impacts from the proposal if it
can be done, and to help the City decide whether an EIS IS
required. An environmental impact statement (EIS) must be
prepared for any proposal with probable significant adverse
Impacts on environmental quality
ThiS environmental checklist asks you to describe some baSIC
information about your proposal The City will use this
checklist to determine whether the environmental impacts of
your proposal are significant and require preparation of an EIS
You must answer each question accurately, carefully and to
the best of your knowledge Answer the questions briefly, but
give the best description you can In most cases, you should
be able to answer the questions from your own observations or
project plans Without the need for experts If you do not know
the answer, or if a question does not apply to your proposal,
write "do not know" or "does not apply" Complete answers to
the questions now may avoid delays later If the space
provided is too small, feel free to attach additional sheets

Lasco Bathware Inc. is proposing the installation of a Regenerative Thermal
Oxidizer at Lasco Bathware's Yelm facility. The equipment will be manufactured
by Airex and resemble the attached drawing. The thermal oxidizer will be installed
in conjunction with an air duct system that enables Lasco to capture the styrene
emissions from one of the spray booths (Gel Coat) in the operation. The thermal
oxidizer controls the emissions from this spray booth which allow the facility
greater flexibility to meet seasonal demand.

NOTICE OF CONSTRUCTION (NOC) APPLICATION INSTRUCTIONS
APPLICATION FE::: SCHEDULE
The fee for processing a Notice of Construction and Application for Approval (NOC) snail Include a Filing Fee
according to Section A, Plan Examination and Inspec:ion Fees according to Section B, and any applicable
Additional NOC Processing Fees according to Section C
A) Filing Fees The Authoritv shall not commence processing of a NOC until It has received a filing fee of

B) Plan Examination and Inspection Fees A Plan Examination and Inspection Fee shail be paid for eac;-,
piece of equipment or process procosed whic:, emits all' pollutants and requires filing a NOe, and for certain fee
eligible reviews and determinations as identified in Table 1 The applicant may choose to determine applicable
Plan Examination and Inspection Fees based on thIS section and include payment along with the NOe application,
or may elect to have the ~uthority cetermine applicable Plan Examination and Inspection Fees during the NOC
compieteness reView, in WhIC;-' case the applicant would be billed In either case, the NOe appiication is
incomolete until the Authority has -eceived payment of applicable Plan Examination and Inspection Fees Plan
Examination and Inspec:ion Fees shail be determined as follows
(1) One Pian c:xamlli2tlOn and inspec:cn Fee shall be paid for each regulatory de!ermlnation or
re'/iew Item Identified in T2ble 1 whic:, appiies to the NOC
(2) One Plan Examln2tion and Inspection Fee shall be paid for each piece of equioment or process
which emits air ;Joilutants and requires filing a NOC except for equipment or processes which can be

(3) E"ulpment or precesses may be conSidered Identical provided that they have .he same phYSiC:::1
spec:ficatlons and only er,e examination 2re/or Inspection IS required by the Authoritv
(4) Identicai equiorrent or processes ;nay be accounted for collectively as a single piece of
equipment or process SL.;DJeC! to a Single pl2n Examination and Inspection Fee,
(5) The Pian Examination and Inspec:ion Fee for a piece of equipment Sh2il be b2sed on the fee
amount in Table 1 which most closely matches the equipment or process type, and,
(6) Any fee basee en actual cost to tr,e Authority shall be determined according to C
C) Additional Fees An F.ccitional NOC Precessing Fee shall be paid by the applicant 701' any work

NOC The Authority shall not Iss~e the Final De!ermination or Order of Approval for any NOC until applicable
additional NOC Processing Fees have been paid The Authority shall determine which additional NOC Processing
Fees apply and sha!1 bill an applicant after issuing a Preliminary Determination, but prior to issuing a Final
Determination or Approval Order Additional NOC Processing Fees shal! be determined based on the fee
schedule contained in Table 2 Any fee based on actual cost to the Authority shall be determined according to C1
(1) Fee amounts in Table 1 and Table 2 whie:, are based on the Authority's actual cost to complete a review
or task shall be determined usjr,~ the actual direct hours expended completing the specific review or task and the
corresponding direct hourly salar; rate of each A.uthority staff person directly involved Tne foilowing provisions
shall apply
(a) Ac~ual hours used in determining the amount of a fee shall be recorded on a daily basis by each
Authority staff person directly involved In completing the specific task,
(b) Time accrued for purposes of determining the amount of a fee for this section shall be accountec
for to the nearest 15 minutes,
(c) Current emplovee salary rates s;,all be used when calculating actual cost-based fees, and,
(d) Tne bill issued for any fee based on the Authority's actual cost shall Indicate the total hours
expended and the hourly cost rates which were used to determine the fee
